Dear Mr. & Mrs. Bennett:
Ron Boone, Seren Homer, Jesse Barnes, and Kristin Potwora, of the NC Division of Water
Resources (DWR), Winston-Salem Regional Office (WSRO), met with you, current owners of
2051 Woodstock Rd in Clemmons, NC, to perform a compliance evaluation inspection of the
home's single family wastewater treatment system, on February 8, 2023. The inspection findings
are summarized below and detailed in the attached inspection report.
- Tax records show you have lived in the house since March, 2001. You currently have no
records of the system.
- You have had the septic tank pumped.
- The location of the chlorinator is unknown and chlorine tablets are not being used to
disinfect the effluent before going to the receiving stream.
- Three separate outfall pipes are located at the stream within approximately 50' of each
other and it is uncertain which one belongs to your home.
- You have not had any laboratory analyses of the effluent performed yet.
Please be aware that the Division may take enforcement action against any permittee who fails to
comply with any requirement of their permit. Pursuant to NC General Statute (NCGS) 143-215.6A,
a civil penalty of not more than twenty-five thousand dollars ($25,000.00) may be assessed against
any person who violates or fails to act in accordance with the terms, conditions, or requirements
of any permit issued pursuant to G.S. 143-215.1.

Dear Mr. and Mrs. Bennett,
Ron Boone, Seren Homer, Jesse Barnes, and Kristin Potwora, of the NC Division of Water Resources
(DWR), Winston-Salem Regional Office (WSRO), met with Mrs. Bennett, current owners of 2051
Woodstock Rd in Clemmons, NC, to perform a compliance evaluation inspection of the home's single
family wastewater treatment system, on February 8, 2023. The inspection findings are summarized
below and in the checklist on the following pages.
- You are the current owners of the residence and the current permittee.
- In accordance with the NCG550000 permit, you must have the septic tank pumped out completely
at least every 5 years, or when the solids level in the tank reaches 1/3 or more of the liquid depth of
the tank. You are aware of the location of the tank and are having it pumped regularly. You should
keep all receipts/invoices for having the tank pumped as part of your file wastewater system at your
home. There were no obvious signs of tank malfunction or problems.
- There were no signs of malfunction or problems with the subsurface sand filter, however, the
filter's location is unknown.
- The chlorinator is missing and cannot be located. The chlorinator should be located or a new one
installed. A septic company can assist with these activities. 2-5/8" calcium hypochlorite tablets are the
correct type of chlorine to use. DO NOT use chlorine tablets intended for swimming pools or similar
applications, of which the main ingredient is usually some type of Chlorinated Isocyanurate. Calcium
hypochlorite is a very potent oxidizer and can cause severe irritation of skin, eyes, etc. Be sure to
wear eye, respiratory, and hand protection when handling the tablets. Do NOT breathe fumes from
the tablets. Thoroughly rinse any body parts that contact the tablets with fresh water.
- There are three outfall pipes at the stream within approximately 50' of each other. Although it is
assumed that yours would be the furthest one to the left when looking at the stream from the back of
the house, it is not certain. A plumbing or septic system company can also assist in determining which
pipe is yours by using a tracer dye. All three of the pipes are readily accessible.
- No laboratory analyses of the effluent have been completed as required by the permit. We
provided you a copy of the general permit and went through its primary requirements with you. You
may find certified laboratories capable of running the required tests for you at this website:

We believe the Pace Labs office in Kernersville is the closest one to your residence.
- As you already know, there is a $60 annual fee for this permit. It is sent to you every year. No
annual fees are overdue at this time. In addition, the permit will be renewed every five years. You will
need to do nothing at this time. You will simply receive a copy of the renewed permit once it is
completed.
- It is important that you maintain your wastewater treatment system in top condition at all times for
several reasons:
o Reduce potential repair costs
o Protect the receiving stream from too much pollution
o Preserve and increase the value of your home by properly maintaining its wastewater disposal
system
o Protect your home from unnecessary and potentially dangerous wastewater backups into the
home, which can cause extensive damage and health hazards
