Dear Mr. Goodman:
A letter of request for an Authorization -to Construct was received July 20,
1988, by the Division and final plans and specifications for the subject project
have been reviewed and found to be satisfactory. Authorization is hereby granted
.for the construction of a 0.5 acre sludge storage lagoon, a sludge loading station
and related pipes and appurtenances as an addition to the existing treatment plant..
This Authorization to Construct is issued in accordance with Part III paragraph
13 of the NPI)ES Permit No. NC0024881 issued November 18, 1987, and shall be
subject to revocation unless the wastewater treatment facilities are constructed in
accordance with the conditions and limitations specified in Permit No. NC0024881.
The Permittee must employ a certified wastewater operator in accordance with
Part III paragraph C of the referenced permit.
The sludge generated from these treatment .facilities must be disposed of in
accordance with G.S. 143-215.1 and in a manner approvable by the North Carolina
Division of Environmental Management.
The Winston Salem Regional Office, telephone number 919/761-2351 shall be
notified at least twenty-four (24) hours in advance of operation of these
facilities. Such notification to the Regional Supervisor shall be made during the
normal office hour from 8:00 a.m. until 5:00 p.m. on Monday through Friday,
excluding State Holidays.
In event the facilities fail to perform satisfactorily in meeting its NPDES
permit effluent limits, the permittee shall take such immediate corrective action as
may be required by this Division, including the construction of additional
wastewater treatment and disposal facilities.

Upon completion of construction and prior to operation of this permitted
:acility, a certification must be received from a professional engineer certifying
: that the permitted facility has been installed in accordance with the approved plans
and specifications. Mail the Certification to the Permits and Engineering Unit,
P.O. Box 27687, Raleigh, N.C. 27611-7687.
Prior to beginning waste disposal operations, two (2) monitor wells, one (1)
upgradient and on (1) downgradient, must be installed to monitor groundwater .
quality. The location and construction details for these wells must be by approval
of the Winston-Salem Regional Office, from which a well construction permit must be
obtained.
The monitor wells must be sampled initially after construction (and prior to
waste disposal operations) and thereafter every March, July, and November for the
following parameters:

Total Coliform
The measurement of water level must be made prior to sampling for the remaining
parameters.
The results of the sampling and analysis shall be sent to the N.C. Division of
Environmental Management of Form GW-59 (Compliance Monitoring Report Form) every
April, August and December.
The lagoon shall have a liner of natural material at least two (2)_geet in
thickness and having a hydraulic conductivity of no greater than 1 x 10
centimeters per second when compacted.
Following installation and inspection of the lagoon liner, and prior to waste
disposal operations, verification of the liner's compliance with thickness and
permeability specifications (via field or laboratory testing), shall be provided to
the Department by the project engineer.
Any additional groundwater quality monitoring, as deemed necessary by the
Department, shall be provided.
Unsuitable soils at the site will be removed to a depth of 15 to 20 feet and
replaced with approved structural fill. Provisions .to control groundwater during
excavation and backfilling will be required. Dewatering of the site shall be
provided if necessary in order to facilitate appropriate liner construction. The
engineer's certification will be evidence that these provisions have been met in the
applicable cases
A copy of the approved plans and specifications shall be maintained on file by
the Permittee for the life of the project.
