Dear Ms. Velasquez:
On March 13, 2023, Jim Gonsiewski of this office visited your property at 4980 US Highway 220
Business to conduct a Technical Assistance Visit. Previously, on February 8, 2023, a Compliance
Evaluation Inspection on the subject wastewater treatment facility permitted at this address was
conducted by Mr. Gonsiewski. At some point prior to the visit on February 8, 2023, the house has
been demolished to its foundation. DWR records show that a septic tank, sand filter, and chlorinator
have been previously described as being located on the site. DWR staff were not able to identify
the precise locations of these features or determine if they still exist and did not find the effluent
outfall during this inspection either. However, staff did observe what may have been a chlorinator
down the hill from the residence. Mr. Gonsiewski was unable to contact you before completing this
inspection.
Records indicate that you are listed as the owner of the permit. On February 27, 2023, this office
sent an email to your address listed in our files. After receiving a response from you on March 10,
2023, the meeting was scheduled at the property for March 13, 2023.
Mr. Gonsiewski met Mr. Luis Gonzalez on March 13 at the site at 5:30 PM. He informed Mr.
Gonsiewski that you will be living in a mobile home on the property. He also stated that the home
that was previously on the property was uninhabitable. They walked around the site and discussed
the various components of the septic system. Mr. Gonzalez had located the septic tank and the
water supply well. Two other components of the system (the chlorinator and effluent pump) were
also located. The exact location of the sand filter and the effluent outfall were not determined. Mr.
Gonzalez said he would be replacing the effluent pump.
There are several operational requirements of the permit that this office wants to bring to your
attention before the system is used. These are listed below.

When the system is operational, you are required to have the discharged water
sampled and analyzed by a certified laboratory annually. Attached please find a list
with the names and phone numbers of two laboratories. Records of monitoring
should be maintained for at least 3 years. At any time, the Division may request to
review documentation required to be kept by the Permit to determine compliance
with its conditions.
As part of the permit, you are required to use the correct tablets in the chlorinator
tubes. The tablets used should contain calcium hypochlorite and the system checked
weekly to ensure tablets are still present. Attached to this report are sheets listing the
proper chlorine type and commercial sources of the chlorine. Once you obtain them,
and the system is in operation, the correct tablets should be placed in the PVC tubes
identified during the inspection. The chlorinator system should be checked weekly to
ensure tablets are still present.
The septic tank should be pumped every three (3) to five (5) years. Please keep the
receipts of these pumping events.
The yearly permit fee of $60 should be paid in a timely manner. You will be billed
for this fee directly from Raleigh.
When the residence is in place and the septic system is operational, please contact this
office and we will conduct a full compliance inspection.
For your information, attached please find copies of the General Permit, the Technical Bulletin, and
the EPA Water Compliance Inspection Report. If you have any questions regarding this matter,

On March 13, 2023 Division of Water Resoources staff Jim Gonsiewski met Mr. Luis Gonzalez, father
of the owner, at the site at 5:30 PM. He informed Mr. Gonsiewski that the owner, Ms. Nataly Gonzalez
Velazquez, will be living in a mobile home on the property. He also stated that the home that was
previously on the property was uninhabitable. Mr. Gonzalez had located the septic tank and the
