SUBJECT: Compliance Evaluation Inspection
NC General Wastewater Permit NCG510000 for Discharge of Wastewater from a
Groundwater Remediation System
Certificate of Coverage: NCG510399 for the Isaac's Tire and Service Center,
Zionville, Watauga County, North Carolina
Dear Ms. Blalock:
Mr. Jim Gonsiewski of the North Carolina Division of Water Resources (DWR), Winston-Salem
Regional Office (WSRO), visited the abovementioned facility on February 28, 2023 and met with
Mr. Trevin Clough, Senior Environmental Technician of WithersRavenel to perform a compliance
evaluation inspection on the facility's groundwater remediation system. Details of that inspection
are included below.
The remediation system currently at the site has been operating since 2016 based on directives by
the NCDEQ Underground Storage Tank (UST) section. It is located in the Creekside Mobile Home
Community. Currently sixteen (16) wells are located on and off site. The monitoring wells are
sampled annually. The effluent is sampled quarterly if flow is coming into the system. A trench
system was installed near the base of the hill at US Highway 421 to intersect the groundwater
moving downgradient from Isaac's Tire and Service Center located east of the site across US
Highway 421. The water collects in the trench and is gravity fed to the remediation trailer for
treatment. The remediation system consists of an oil -water separator, an air stripper, a 25-micron
bag filtration system, a carbon filtration system, and an underground batch tank. The remediation
system appeared to be well maintained.
The effluent sample collection point is located in the remediation trailer near the point where it exits
the trailer to the holding tank located adjacent east of the system. The outfall for the 2-inch PVC
effluent pipe that transports treated water from the remediation system by gravity is located west of
the site. This outfall empties into an unnamed tributary to Sharp Creek. The system effluent was
last sampled on November 16, 2022.
Effluent laboratory analytical results were reviewed from October 28, 2020 to November 16, 2022.
It was noted during the inspection that lead was not reported in the results from the sample collected

on September 1, 2021. Additionally, the detection limits for lead exceeded the NPDES Daily
Effluent Maximum of 3 ug/L in the samples collected on October 28, 2020, February 23, 2022,
September 7, 2022, and November 16, 2022. The detection limit for toluene exceeded the NPDES
Daily Effluent Maximum of 0.36 ug/L in all samples collected. Please ensure that all required
constituents are analyzed and reported and that laboratory detection limits do not exceed the
NPDES Daily Effluent Maximums, per the permit requirements.
