If so, I need him to initial the form to signify what he is doing and what documents he has. There are options for: A)
Renewal Only; B) Renewal and Modification; C) Renewal and a transfer or renewal and a project name change; or D) All
of the above.
In addition, Section IV of the document was not completed. The permitted BMP is a wet pond.
The permit was already transferred and modified in 2014, the recorded deed restrictions were done in April 2014, the
designers certification was submitted in January 2015, so Mr. Haberman should be able to complete all of the A.
Renewal Only section of the form.
