On October 11, 2007, the Express Review Program of the Division of Water Quality (DWQ) received additional
information from Mr. Jay Keller of Keller Environmental. The DWQ has determined that this information is
incomplete. Please provide the following information so that we may continue to review your project and
prevent denial of your application as required by 15A NCAC 2H .0506:
1. Please resubmit a PCN form to accurately reflect the stream, wetland, and buffer impacts shown on the plans.
Section III,. bullet 5 (attached sheet) does not match impacts portrayed on the remainder of the form. Please
revise the following:
a. Section III, bullet 10
b. Section VI, written description
c. Section VI, Wetland table (Impact 4 should be 0.12)
d. Section VI, Stream table
i. Aerial crossing at Impact 3 .has 30 feet of temporary impact?
ii. Impact 6 should be a permanent impact.
iii. Impact 7 should be 0.003 acres.
e. Section VI, Cumulative impact table (to reflect revisions).
f. Section X, Riparian Buffer table (Zonel and Zone 2 impacts) and remove required mitigation.

2. Please revise. Plan sheet D-3 to incorporate flange and anti-seep collar detail. NRCS detail provided in Oct.
11, 2007 submittal is not signed or dated (it should be included in official plans).
Please respond within five (5) days of the date of this letter by sending three (3) copies of this information. The
Express Review Program is a process that requires all parties to participate in a timely manner.
This letter only addresses the application review and does not authorize any impacts to wetlands, waters or protected
