Dear Mr. Routh:
A letter of request for an Authorization to Construct was received on September 15, 1990,
by the Division and final plans and specifications for the subject project have been reviewed and
r. found to be satisfactory. Authorization is hereby granted for the construction of modifications to
the wastewater treatment facilities which will increase the capacity to 13.5 MGD and will consist of
the construction of a chemical feed building and a chlorination building the installation of polymer
feed equipment including two 10,000 gallon heated insulaiedltor oP ran c two metering pumps,
two polymer b ending systems, on s c mrer, sodium bisulfite feed equipment, a 7500 gallon
heated insulated storage tank, two automatic chlorinators, a manual chlorinator, an automatic
switchover unit, a turbine type mixer, a return sludge splitter box, mixed liquor splitter boxes 1
and 2, final settling tanks 3 and 4, return sludge pump station No. 2, a sludge thickener, a plant
drain pump station, chlorine contact tanks 2 and 3, modifications to the existing influent structure
and the operation building, the installation of eight floating aerators and three mechanical mixers in
two existing aeration basins, modifications to sludge return pump station No. 1, modifications to
sludge drying beds, modifications to final settling tanks 1 and 2, the installation of a mechanically
cleaned bar screen, two catenary bar screens, two fixed bar screens, a chain and bucket type grit
collector, the installation of two rotary positive displacement blowers, modifications to two
existing 90 foot diameter tanks and two existing 130 foot diameter tanks to include the removal of
existing sludge collection equipment, the installation of a center pier support, a siphon feed, and
peripheral effluent suction type sludge collector, the installation of two dissolved air flotation type
sludge thickeners in 16 foot by 60 foot concrete tanks, the installation of pumps, piping, valves,
and appurtenances with the discharge of treated wastewater into receiving waters designated as the
Dan River in the Roanoke River Basin.
This Authorization to Construct is issued in accordance with Part III paragraph C of
NPDES Permit No. NCO025071 issued November 15, 1990, and shall be subject to revocation
unless the wastewater treatment facilities are constructed in accordance with the conditions and
OOWA limitations specified in Permit No. NCO025071.

J'� The sludge generated from these treatment facilities must be disposed of in accordance with
G.S. 143-215.1 and in a manner approved by the North Carolina Division of Environmental
011;_ Management.
In the event that the facilities fail to perform satisfactorily, including the creation of
nuisance conditions, the Permittee shall take immediate corrective action, including those as may be
required by this Division, such as the construction of additional or replacement wastewater
treatment or disposal facilities.
The Winston-Salem Regional Office, phone no. 9191761-2351 shall be notified at least
forty-eight (48) hours in advance of operation of the installed facilities so that an in -place
inspection can be made. Such notification to the regional supervisor shall be made during the
normal office hours from 8:00 a.m. until 5:00 p.m. on Monday through Friday, excluding State
Holidays.
Upon completion of construction and prior to operation of this permitted facility, a
certification must be received from a professional engineer certifying that the permitted facility has
been installed in accordance with the NPDES Permit, this Authorization to Construct, and the
approved plans and specifications. Mail the Certification to the Permits and Engineering Unit,
P.O. Box 27687, Raleigh, NC 27611.
The Permittee shall employ a certified wastewater treatment plant operator to be in
responsible charge of the wastewater treatment facilities. The operator must hold a certificate of the
grade at least equivalent to the classification assigned to the wastewater treatment facilities by the
Certification Commission.
A copy of the approved plans and specifications shall be maintained on file by the Permittee
for the life of the facility.
The sand media of the sludge drying beds must comply with the Division's sand
specifications. The engineer's certification will be evidence that this certification has been met.
Failure to abide by the requirements contained in this Authorization to Construct may
subject the Permittee to an enforcement action by the Division of Environmental Management in
accordance with North Carolina General Statute 143-215.6.
The issuance of this Authorization to Construct does not preclude the Permittee from
complying with any and all statutes, rules, regulations, or ordinances which may be imposed by
other government agencies (local, state, and federal) which have jurisdiction.
