This NPDES permit is scheduled to expire on April 30, 2019. The enclosed application is for the renewal of this permit. In accordance with the requirements of federal (40 CFR 122) and state (15A NCAC 2H .0105(3)) regulations, we are submitting three signed copies of the completed application package and associated attachments and figures. The application package includes the following information: • NPDES Permit Application — EPA Form 2A • EPA Form 2A Additional Information (Topographic Map, Site Map, Process Flow Diagram and Process Narrative) • Terrible Creek WWTP Sludge Management Plan • Summary of First Species Effluent Toxicity Testing, 2015-2018 • Annual Priority Pollutant Analysis Results, 2015-2018 • Technical Memorandum in support of the Reduction of Monitoring Frequency for Exceptionally Performing Facilities At the time of this application submittal, the required second species toxicity testing events have not yet been completed. The four second species effluent toxicity tests are currently scheduled to be completed in October 2018, November 2018, December 2018, and January 2019. When the toxicity test results become available, they will be forwarded to the Division of Water Resources (DWR) along with revisions to Part D of EPA Form 2A to include these results. All of the second species toxicity tests will be submitted prior to the current permit expiration date of April 30, 2019. In accordance with 15A NCAC 2B .0508(b)(1) and the October 2012 DWR Guidance Document for the Reduction of Monitoring Frequency for Exceptionally Performing Facilities, the Town of Fuquay-Varina respectfully requests a reduction in frequency monitoring for biochemical oxygen demand (BOD5), total suspended solids (TSS), and fecal coliform bacteria. Effluent sampling results from the past three years demonstrate that all state regulatory and guidance requirements have been met in support of this request. A complete summary of the sampling data and analysis for the reduction of monitoring frequency is attached in Tab G of this application. Discharges an average of 25,000 gallons per day or more of process wastewater to the treatment works (with certain exclusions); or b. Contributes a process wastestream that makes up 5 percent or more of the average dry weather hydraulic or organic capacity of the treatment plant; or C. Influent Pumping and Preliminary Treatment The Terrible Creek Wastewater Treatment Plant (WWTP) operates an influent pump station with a total of six (6) submersible pumps and a firm pumping capacity of 9 mgd. The influent pump station receives flow from a 24-inch gravity main into an influent channel, which then passes through a mechanically cleaned coarse bar screen. Following the coarse screen flow can be split between two influent wet wells, each of which contain two larger capacity pumps (40 HP) and one smaller capacity pump (25 HP). The WWTP influent is then pumped through a single 18-inch force main to the preliminary treatment facility (PTF), which includes fine screens and grit removal processes. Upon entering the PTF, flow is split between two screenings channels with 1/4-inch mechanically cleaned step screens. A third influent channel with a manually cleaned 1-inch bar rack is also available. After passing through the screenings channels, flow is then directed through two Eutek Headcell stacked plate style grit removal cells, ten (10) six-foot diameter trays. After passing through the grit collection cells, flow is then conveyed to a distribution box, which splits flow between two circular biological nutrient removal (BNR) basins. Secondary Treatment Secondary treatment is provided through two 125-foot diameter, circular, 5-stage conventional BNR basins. Flow enters the basin in the center, which is an anaerobic zone. Following the anaerobic zone, flow goes through the first anoxic zone in an intermediate annular ring, an aerobic zone, a second anoxic zone, and finally a reaeration zone in the outer annular ring of the basin. Fine bubble diffusers are used for aeration and platform mounted mixers are used in the anaerobic and anoxic zones. Window pumps are used for recycling nitrified mixed liquor from the aerobic zone to the first anoxic zone. Following the BNR basins, flow is conveyed to a distribution box, where it is distributed to the two secondary clarifiers. There are two 85-foot diameter, 16-foot sidewall depth secondary clarifiers. The clarifiers are upfitted with suction header type sludge removal systems and rotating trough style scum removal. As part of the activated sludge process, settled activated sludge from the secondary clarifiers is recycled to the influent of the aeration basins (return activated sludge or RAS). A RAS pump station pumps RAS from the two clarifiers to the PTF effluent piping, upstream of the BNR Town of Fuquay-Varina Terrible Creek WWTP NPDES Permit No. SC0066516 Outfall 001 basin distribution box. Secondary clarified effluent is conveyed to the WWTPs tertiary filtration system. The Terrible Creek WWTP has the capability to feed three different chemicals to assist in the WWTP treatment processes. The plant has the ability to feed soldium hydroxide (caustic) to primary effluent upstream of the BNR basins to provide supplemental alkalinity for nitrification. Supplemental carbon can be added in the BNR basins as an external source of readily biodegradable carbon for heterotrophic denitrifying bacteria if needed. Alum is capable of being added upstream of secondary clarifiers for chemical phosphorous removal. Filtration & Disinfection Secondary clarified effluent is conveyed to the WWTP's two Leopold Ultrascreen disk style filtration units. Each filter unit consists of stainless steel mesh media mounted on circular disks, through which forward flow must pass. The filter units each contain 12 stainless steel media mounted disks. The disks rotate during filtration and provide an effective filtration pore size of 10 micrometers (µm). After passing through the filter media, filtered effluent is then combined and conveyed to the WWTP UV disinfection system. The UV disinfection system consists of two UV disinfection channels, each with two service and one redundant bank of UV lamps. The UV system is a TrojanUVSigna System with eight UV lamps per bank. Following UV disinfection, treated effluent is conveyed through flow measurement and effluent sampling and ultimately to the outfall at Terrible Creek. Solids Treatment Residuals generated during the liquid treatment process are removed from the liquid treatment train to maintain an equilibrium concentration of activated sludge solids in the biological treatment tanks. The excess residuals or waste activated sludge (WAS) are removed from the liquid treatment process in the return activated sludge stream. WAS is capable of being pumped from the WAS pump station to either a sludge holding tank or a gravity belt thickener. The sludge holding tank receives sludge from the WAS pump station and the thickened WAS (TWAS) pumps when the dewatering unit is operating in thickening mode. WAS is removed from the sludge storage tank by the sludge feed pumps located in the solids handling facility. The sludge storage tank has a 61.5-ft diameter by 18-ft sidewater depth with a volumetric capacity of approximately 400,000 gallons. Aeration and mixing capability is proposed via a fine bubble membrane diffuser system. An aeration rate of 20 cfm per 1000 cubic feet keeps solids aerated and in suspension. The Terrible Creek WWTP solids dewatering facility provides dewatering of WAS. The WWTP is currently operating under a permitted discharge flow of 3.0-mgd. The solids handling processes at the Terrible Creek WWTP: • Provide raw waste activated sludge thickening by utilization of two "3-belt" dewatering units capable of independent operation as a gravity belt thickener which can produce a thickened waste activated sludge product. Liquid residuals storage for this material is provided via an interconnection with the sludge storage tank. Additionally, thickened waste activated sludge product can also be routed to a liquid loadout facility. • Provide process capability to produce a belt filter press dewatered cake product from the treatment facility. The dewatered cake produced is then routed to off -site composting facilities (McGill Environmental, New Hill, NC). Trash, Sediment and Grit The Terrible Creek WWTP process design includes coarse (influent pump station) and fine (preliminary treatment facility) screens for the removal of trash and sediment from the liquid treatment trains. The preliminary treatment facility also includes a grit removal system composed of a grit concentrator and grit washing/classification unit. Both the influent pump station and the preliminary treatment facility are designed with screenings collection capabilities for short-term storage, prior to being transported to an offsite landfill facility. Estimated Residuals Production Rates Undigested (raw) residuals production rates were estimated for the facility based on a BioWin® process model developed for the WWTP. The BioWin® process model utilized historical influent BOD, TSS, and other wastewater characteristics in developing estimates for treatment facility process performance and residuals production rates. The estimated residuals production rates for the facility are as shown in Table 1. Waste Activated Sludge Pumping Waste activated sludge has the same solids concentration as the plant's return activated sludge (e.g., secondary clarifier underflow concentration). Waste activated sludge branches off of the RAS pump discharge header inside the RAS pump station building. The WAS flow is measured with a magnetic flow meter and controlled using a motor operated modulating plug valve. The WAS is routed to the sludge storage tank for storage or to the dewatering building for thickening or dewatering. Sludge Storage Raw waste activated sludge storage is provided in a 61.5-ft diameter x 18-ft sidewater depth holding tank with a volumetric capacity of approximately 400,000 gallons. Aeration and mixing are provided in this tank by fine bubble membrane diffusers. Underflow from this tank is routed to the dewatering facility where raw waste activated sludge can be: (1) further thickened and sent back to the sludge storage tank (recuperative thickening operations mode) or liquid loadout; or (2) fully dewatered and routed to off -site management facilities (e.g., off -site composting). Under normal operating conditions, the Terrible Creek WWTP design provides for 6.5 days of residuals storage for un-thickened WAS at 7,000 mg TSS/L concentration. The solids processing design also includes provisions to increase the residual storage capacity at the plant to greater than 30-days when required, by thickening the sludge with a gravity belt thickener prior to storage in the sludge storage tank and/or the use of the gravity belt thickening process in a recuperative thickening operational mode. The methods and derivation of the design sludge storage capacity under both normal operating and during periods where additional residual storage capacity is required are outlined in the paragraphs below. After being processed through the gravity belt thickener, the thickened sludge is directed to the TWAS pump station instead of continuing to the belt filter press. The thickened sludge is then pumped by the TWAS pumps to the sludge storage tank. The solids processing system was designed to allow for greater than 30-days of residual sludge storage when operating conditions dictate. The gravity belt thickener is designed to process sludge with up to a 4% solids concentration. Both the Sludge Feed and TWAS pumps are also designed to allow for pumping sludge with a 4% solids concentration. The Terrible Creek WWTP residual storage design allows for the storage of thickened WAS up to a 4% solids concentration. The calculations demonstrate that greater than 30-days of onsite residual storage is available at the WWTP, in accordance with 15A NCAC 02T .0505(o), when the solids concentration of the sludge stored in the sludge storage tank is raised to 3.5%. When solids concentrations exceed 2.5% in the sludge storage tank, aeration of the tank will cease until normal operating conditions resume. The lack of aeration will not have an impact on the storage of sludge in the tank. The units are "3-belt" dewatering units which are characterized by an independent gravity belt thickener coupled with a conventional 2-belt belt filter press dewatering unit. The 3-belt system provides the capability to operate in either a "thickening" or "dewatering" dual duty operating configuration. The 3-belt units provide the capability to produce a dewatered cake product which can be transported off -site for management and/or disposal. Thickened waste activated sludge pumps are provided in the solids handling building to transfer thickened material from the independent gravity deck to the sludge storage tank or to the liquid loadout station. Liquid Sludge Loading and Handling The liquid loading station is fed from the TWAS pumps located in the dewatering building. Capability is provided to allow liquid loading of un-stabilized materials for off -site management. The monitoring frequency reduction analysis was performed in accordance with the Division of Water Resources (DWR) guidance document, Reduction of Monitoring Frequencies in NPDES Permits for Exceptionally Performing Facilities (October 2012). Per DWR guidance, the following criteria has been met by the Town of Fuquay-Varina: • The Terrible Creek WWTP has not had any civil penalty assessments for permit limit violations for either biochemical oxygen demand (BOD5), total suspended solids (TSS), or fecal coliform. • The Town of Fuquay-Varina and Terrible Creek WWTP employees have not been convicted of any criminal violations of the Clean Water Act (CWA). • The Terrible Creek WWTP is not currently under a Special Order of Consent (SOC). • The Terrible Creek WWTP is not on EPA's Quarterly Noncompliance Report for either BOD5, TSS, or fecal coliform. Table 1 provides a summary of DMR data from August 2015 through July 2018. The data supports a reduction in the monitoring frequency for BOD5, TSS, and fecal coliform. The three-year arithmetic mean of effluent data is less than 50 percent of the monthly permit limit for BOD5 and TSS. The three-year geometric mean of effluent data is less than 50 percent of the monthly permit limit for fecal coliform. Less than 15 of the daily effluent sampling results are over 200 percent of the monthly average limit for BOD5 and TSS. Less than 20 of the effluent sampling results are over 200 percent of the weekly average limit for fecal coliform. The Excel Spreadsheet, NPDES Permit NCO066516 MRF.xlsx, contains the data for the monitoring reduction frequency analysis. This spreadsheet will be transmitted to DWR upon request. The Terrible Creek WWTP has met the requirements to be categorized as an Exceptionally Performing Facility per DWR guidance and is eligible for a reduction in the monitoring frequency of BOD5, TSS, and fecal coliform bacteria.
