Hi David,
I have completed an initial review of the 401 application for the subject project and require additional information to
complete the review. Please provide a response to the below items within 30 days. Please note that the application will
be considered “on hold” until a complete response is received.
1. On pages 16 and 17 of the PCN, it was indicated that a stormwater management plan (SMP) had been approved,
but no answers were provided on which stormwater management program area approved the plan, and if local,
what stormwater program it is in. Additionally, Guilford County is listed on page 17 of the PCN as being the local
government stormwater program.
The site is low density. The SMP has not been approved at this time. It will be under state program for Phase II
in Stokes County. The SMP will be submitted later this week to DEQ and separately from this PCN for review and
approval.
Please correct the information for which agency has approved the SMP and please provide the approved SMP.
Revised pages 16 and 17 are attached.

2. Are lots 3, 16, and 17 planned to be buildable lots and how does the project plan to protect the stream in these
areas?
The lots mentioned are planned residential lots for single family homes. The stream has a local requirement for
30’ stream buffer and all EC and NCG01 requirements will apply during the infrastructure and grading phase of
work. EC plans will be provided for review and approval to the Winston-Salem DEQ after 401 certification has
been received. Based on conversations between the civil designer and Ms. Tamera Eplin, the State 401 and
USACOE 404 permits are required to process the EC submittal. A separate EC plan, prepared by the builder, for
lot specific controls will be required along with NCG01 certification for the home construction phase of
work. The SMP will provide diffuse flow to meet state requirements under low density rules.
