Dear Mr. Ray:
Division personnel have reviewed and approved your application for renewal of the subject permit.
Accordingly, we are forwarding the attached NPDES discharge permit. This permit is issued pursuant
to the requirements of North Carolina General Statute 143-215.1 and the Memorandum of Agreement
between North Carolina and the U.S. Environmental Protection Agency dated October 15, 2007 (or as
subsequently amended).
This final permit includes no significant changes from the draft permit sent to you on July 18, 2012.
If any parts, measurement frequencies or sampling requirements contained in this permit are
unacceptable to you, you have the right to an adjudicatory hearing upon written request within thirty
(30) days following receipt of this letter. This request must be in the form of a written petition,
conforming to Chapter 150B of the North Carolina General Statutes, and filed with the Office of
Administrative Hearings (6714 Mail Service Center, Raleigh, North Carolina 27699-6714). Unless
such demand is made, this decision shall be final and binding.
Please note that Beaverdam Creek is listed as an impaired waterbody on the North Carolina 303(d)
Impaired Waters List. Addressing impaired waters is a high priority with the Division, and instream
data will continue to be evaluated. If there is noncompliance with permitted effluent limits and stream
impairment can be attributed to your facility, then mitigative measures may be required
Please also note that this permit is not transferable except after notice to the Division. The Division
may require modification or revocation and reissuance of the permit. This permit does not affect the
legal requirements to obtain other permits which may be required by the Division of Water Quality or
permits required by the Division of Land Resources, the Coastal Area Management Act or any other
Federal. or Local governmental permit that may be required.

In compliance with the provision of North Carolina General Statute 143-215.1, other lawful standards

Bethel Elementary School WWTP

to receiving waters designated as Beaverdam Creek in the Watauga River Basin
in accordance with effluent limitations, monitoring requirements, and other conditions set forth in Parts
I,11, Ill and IV hereof.
This permit shall become effective February 1, 2013.
This permit and authorization to discharge shall expire at midnight on September 30, 2017.

SUPPLEMENT TO PERMIT COVER SHEET
All previous NPDES Permits issued to this facility, whether for operation or discharge are hereby
revoked. As of this permit issuance, any previously issued permit bearing this number is no longer
effective. Therefore, the exclusive authority to operate and discharge from this facility arises under the
permit conditions, requirements, terms, and provisions included herein.
The Watauga County Board of Education
is hereby authorized to:
1. Continue to operate an existing 0.0065 MGD extended -aeration wastewater treatment system with
the following components:
• Grit chamber
• Aeration basin
• Clarifier
• Chlorination
• Dechlorination
• Return sludge holding tank
This facility is located at the Bethel Elementary School WWTP, 138 Bethel Road, in Bethel, in
Watauga County.
2. Discharge from said treatment works, through Outfall 001, into Beaverdam Creek, classified C-
Trout water in the Watauga River Basin, at the location specified on the attached map.

ed renewal of permit NCO067024 for the Valle Cru-
cis Elementary School WWTP in Watauga County;
this Permitted discharge is treated wastewater to
Dutch Creek in the Watauga River basin.
The Watauga County Board of Education request-
ed renewal of permit NCO066991 for the Bethel Ele-
mentary School WWTP in Watauga County; this
permitted discharge is treated wastewater to
Beaverdam Creek in the Watauga River basin.
Water Quality Utilities, Inc. requested renewal of
permit NCO050610 for The Ponds WWTP in Watau-
ga County; this permitted discharge is treated do-
mestic wastewater to the Watauga River in the
Watauga River basin.
Seven Devils Resort WWTP, NCSR 1151, Seven
Devils, Watauga County, applied to renew permit
NC0035149, discharging to a UT of the Watauga
River, Watauga Basin.
Water Quality Services, Inc. requested renewal of
permit NCO032212 for the Yonahlossee WWTP in
Watauga County; this permitted discharge is
treated domestic wastewater to Lance Creek in
the Watauga River basin.
Carolina Water Service, Inc. of North Carolina re-
quested renewal of permit NCO032123 for the
Hound Ears WWTP; this permitted discharge is
treated domestic wastewater into the Watauga
River in the Watauga River Basin.
Town of Beech Mountain applied to renew NPDES
permit NCO088099 for the Buckeye Lake WTP, 1400
Pine Ridge Road, Watauga County, discharging
treated wastewater to Buckeye Creek, Watauge
River Basin.
Cove Creek Preservation and Development, Inc.
requested renewal of permit N00067008 for the
Old Cove Creek School WWTP in Watauga County;
this permitted discharge is treated wastewater to
Cove Creek in the Watauga River basin.

Attached please find NPDES renewal applications for Bethel Elementary School
WWTP (NPDES Permit # NCO066991) and Valle Crucis Elementary School WWTP
(NPDES Permit # NC0067024).
Watauga County School System's manages the sludge produced from our WWTP
through the use of a local vendor. We pump the sludge as required by volume and each
summer we have all our plants pumped so that we start each school year with little to no
sludge in the holding tanks.
