In accordance with the application for a discharge permit received on March 13, 1997, the Division is
forwarding herewith the subject NPDES permit. This permit is issued pursuant to the requirements of North
Carolina General Statute 143-215.1 and the Memorandum of Agreement between North Carolina and the US
Environmental Protection Agency dated December 6,1983.
If any parts, measurement frequencies or sampling requirements contained in this permit are
unacceptable to you, you have the right to an adjudicatory hearing upon written request within thirty (30)
days following receipt of this letter. This request must be in the form of a written petition, conforming to
Chapter 150B of the North Carolina General Statutes, and filed with the Office of Administrative
Hearings, Post Office Drawer 27447, Raleigh, North Carolina 27611-7447. Unless such demand is made, this
decision shall be final and binding.
Please take notice this permit is not transferable. Part 11, EA. addresses the requirements to be
followed in case of change in ownership or control of this discharge.
This pern-dt does not affect the legal requirements to obtain other permits which may be required by
the Division of Water Quality or permits required by the Division of Land Resources, Coastal Area
Management Act or any other Federal or Local governmental pern-dt that may be required.

This is a permit renewal for a discharge' into Beaverdam Creek in the Watauga,
River Basin. Within the past twenty four months this facility had one violation
of BOD5., Downstream temperature values were above 20'C only during
August of 1995. Fecal Coliform seems to be of some concern. The facility
inconsistantly discharged levels above 200/100 ml of fecal colifonn within the
past twenty four months. Downstream levels for fecal coliforrn were also
inconsistantly high within the past twenty four months. Fecal levels were very
high in early 1995. Permits and Engineering will recommend existing limitations
with monitoring reflective of a class II facility. Regional comments must include
the need for this facility to have disinfection and fecal limits. Region must also
verify facility class and components.

Thank you for keeping the monitoring frequencies at 2/month . No objections to
permit Emits proposed. I agree with the proposed regulation of fecal coliform in the
event violations occur. This facility will be used sporadically for classes, since it is no
longer an actual year round school. No problems are anticipated with fecal coliform.
PSI Properties (Four Seasons -Pebble Creek Apartments

The sign on the road in front of the apartments is now Pebble Creek. No change in
location has occurred since the plant was buflt. See staff report and rating sheet dated
4-18-97. This facility does not have UV disinfection.
The plant consists of two aerated influent holding basins, bar screen, aeration basin,
clarifier, chlorine contact basin and dechlorination unit.

Is this facility being constructed with Construction Grant Funds or are any public monies involved. No.
2. Special monitoring or limitations (including toxicity) requests:
We request that the temperature requirement be reduced to weekly effluen frequency. The daily
requirement is not necessary for a Grade H plant. The data shows stream temperature is not impacted
due to the discharge.
3. Important SOC, JOC or Compliance Schedule dates: (Please indicate). None.
4. Alternative Analysis Evaluation: Has the facility evaluated all of the non -discharge options available.
Please provide regional perspective for each option evaluated. NA because discharge is to a positive
flow stream.

PART IV - EVALUATION AND RECOMMENDATIONS
An evaluation of the self -monitoring data submitted for the period of December 1995 through November 1996
reveals compliance with all monthly average limits, except for one month when BOD5 was violated.
Recommendation is made to renew the permit in accordance with division policy.
