Director Robin Jarvis, Owner 5600 Styers Ferry Rd Clemmons, NC 27012 NORTH CAROLINA Environmental Quality March 21, 2023 Subject: Compliance Evaluation Inspection Single Family Residence Wastewater Treatment System 5600 Styers Ferry Rd, Clemmons, NC 27012 General NPDES Wastewater Permit: NCG550000 Certificate of Coverage: NCG550749 Forsyth County Dear Ms. Jarvis: Ron Boone, of the Winston-Salem Regional Office (WSRO) of the NC Division of Water Resources (DWR or the Division), conducted a follow-up compliance evaluation inspection of your single family residence wastewater treatment and disposal system covered by discharge permit NCG550749 on March 6th, 2023. On March 6th, Mr. Boone met with you at which point you informed him that your system is a conventional septic system with a subsurface drainage field and does not discharge. You also stated that the home was originally built with a conventional septic system, which failed a few years later, at which point you applied for and received this discharge permit. However, before installing the discharging system, the septic contractor suggested to install additional subsurface drainage lines rather than the discharging system. This was done and the system has worked properly as a non - discharging conventional septic system with a subsurface drainage field, ever since. No discharging system has ever been installed at this address, but you maintained the permit and paid all the annual fees in order to keep the permit just in case it was needed in the future. You have now decided the permit is no longer needed and requested via email to the Division that the permit be rescinded. As such, the permit has been rescinded effective 3/7/2023. You should not receive any more annual bills and we will no longer need to perform inspections.
