Dear Mr. Monroe:
In accordance with your request received March 20, 1.987, the
Division of. Environmental Management hereby amends Authorization to
Construct NC0055913, issued November 5, 1.985, as follows:
1.. Combine the two filter boxes into one large box that contains
a dual bed filter;
2. Reduce the number of blowers from four to three of larger
capacities; and
3. Delete the froth control system and bar screen.
This amendment does not change any other conditions of the
original Authorization to Construct. In addition, all the requirements
,,contained in the May 1, 1987, inspection letter to you from our
Winston-Salem Regional Office must be met.

Wastewater Treatment Facilities
Guilford County
Dear Mr. Monroe:
A letter of request for Authorization to Construct was received May 28, 1985,
by the Division and final plans and specifications for the subject project have been
reviewed and found to be satisfactory. Authorization is hereby granted for the con-
struction of a 0.035 MGD wastewater treatment facility consisting of a flow splitter
box, influent bar screens and comminutors, two (2) 26,000 gallon capacity aeration
tanks, two (2) 3,375 gallon capacity clarifiers, two (2) 2,500 gallon capacity sludge
holding tanks, four (4) blowers capable of delivering 110 cfm each, a 35 square foot
tertiary treatment unit, two (2) 350 gallon capacity chlorine contact tanks with tablet
chlorinators, an air diffuser located in the clearwell of the tertiary treatment unit
for post aeration requirements, an effluent recording flow meter, a standby generator
and all associated valves, piping and appurtenances to serve Monroe's Mobile Home Park
located in Guilford County.
This Authorization to Construct is issued in accordance with Part III paragraph
C of NPDES Permit No. NC0055913 issued April 6, 1984, and shall be subject to revoca-
tion unless the wastewater treatment facilities are constructed in accordance with
the conditions and limitations specified in Permit No. NC0055913.
The Permittee must employ a certified wastewater operator in accordance with
Part III paragraph D of the referenced permit.
The sludge generated from these treatment facilities must be disposed of in
accordance with G.S. 143-215.1 and in a manner approvable by the North Carolina
Division of Environmental Management.

The Winston Salem Regional Office, telephone number 919/761-2351 shall be noti-
fied at least twenty-four (24) hours in advance of operation of the system so that
an in -place inspection can be made. Such notification to the Regional Supervisor
shall be made during the normal office hours from 8:00 A.M. until 5:00 P.M. on
Monday through Friday, excluding State Holidays.
In event the facilities fail to perform satisfactorily in meeting it's NPDES
permit effluent limits, the Permittee shall take such immediate corrective action
as may be required by this Division, including the construction of additional waste-
water treatment and disposal facilities.
The wastewater flowrate discharged to and treated by these facilities shall
not exceed 30,000 GPD until such time as NPDES Permit No. NCO055913 is modified to
allow the discharge of up to 35,000 GPD of adequately treated wastewater.
