Division personnel have reviewed and modified parts of your existing NPDES Permit. These modifications are the same
ones addressed in your February 29, 2012 Draft letter, which were:

• Added Condition #2 to the Supplement to Permit Cover Sheet
• Amended A. (1) Effluent Limitations and Monitoring Requirements language

Permittee is authorized to discharge from Outfall 001. Such discharges shall be limited and monitored by
the Permittee as specified below:
We are forwarding you this revised permit with these changes. This permit modification is issued pursuant to the
requirements of North Carolina General Statute 143-215.1 and the Memorandum of Agreement between North Carolina
and the U.S. Environmental Protection Agency dated October 15, 2007 (or as subsequently amended). Please note that
this permit is not transferable except after notice to the Division. The Division may require modification or revocation and
reissuance of the permit. This permit does not affect the legal requirements to obtain other permits which may be
required by the Division of Water Quality or permits required by the Division of Land Resources, the Coastal Area
Management Act or any other Federal or Local governmental permit that may be required. If you have any questions

SUPPLEMENT TO PERMIT COVER SHEET
All previous NPDES Permits issued to this facility, whether for operation or discharge are
hereby revoked. As of this permit issuance, any previously issued permit bearing this number
is no longer effective. Therefore, the exclusive authority to operate and discharge from this
facility arises under the permit conditions, requirements, terms, and provisions included
herein.
The Southeastern Wayne Sanitary District is hereby authorized to:
1. Continue to operate a conventional water treatment plant with a filter backwash discharge.
The facility is located at the Devil's Woodyard WTP at 4820 Arrington Bridge Road, near
Williams in Wayne County.
2. After Authorization to Construct and submittal of signed Engineering Certification, to begin
operating an expanded Water Treatment Plant facility.
3. Discharge from said treatment works at the location specified on the attached map into a
Horspen Branch, classified C-Swamp waters in the Cape Fear River Basin.

A. (1.) EFFLUENT LIMITATIONS AND MONITORING REQUIREMENTS
During the period beginning on the effective date of this permit, approved expansions and
lasting until expiration, the Permittee is authorized to discharge from outfall 001. Such

Footnotes:
1) The Division shall consider all effluent TRC values reported below 50 ug/I to be in compliance with the permit.
However,the Permittee shall continue to record and submit all values reported by a North Carolina certified
laboratory(including field certified),even if these values fall below 50 ug/I.
2) Parameter should be monitored in conjunction with toxicity test
3) Chronic Toxicity Monitoring P/F Quarterly TGE6C. See Condition A. (2.). Further studies will be required to
determine the cause of toxicity, if any. The collection of samples shall coincide with the sampling of other
parameters.
There shall be no discharge of floating solids or visible foam in other than trace amounts

A. (2.) CHRONIC TOXICITY MONITORING (QRTRLY)
The permittee shall conduct quarterly chronic toxicity tests using test procedures outlined in the"North
Carolina (Ceriodaphnia Dubia) Chronic Effluent Bioassay Procedure," Revised February 1998, or
subsequent versions.
The effluent concentration defined as treatment two in the procedure document is 90%. The testing shall be
performed as a Ceriodaphnia dubia 7day pass/fail test. The tests will be performed during the months of
January,April,July and October. Effluent sampling for this testing shall be performed at the NPDES
Permitted final effluent discharge below all treatment processes.
All toxicity testing results required as part of this permit condition will be entered on the Effluent Discharge
Monitoring Form (MR-1)for the month in which it was performed, using the parameter code TGP3B.

Completed Aquatic Toxicity Test Forms shall be filed with the Environmental Sciences Section no
later than 30 days after the end of the reporting period for which the report is made.
Test data shall be complete and accurate and include all supporting chemical/physical measurements performed
in association with the toxicity tests, as well as all dose/response data. Total residual chlorine of the effluent
toxicity sample must be measured and reported if chlorine is employed for disinfection of the waste stream.
Should there be no discharge of flow from the facility during a month in which toxicity monitoring is required, the
permittee will complete the information located at the top of the aquatic toxicity(AT)test form indicating the
facility name, permit number, pipe number, county, and the month/year of the report with the notation of"No
Flow" in the comment area of the form. The report shall be submitted to the Environmental Sciences Section at
the address cited above.
Should the permittee fail to monitor during a month in which toxicity monitoring is required, then monthly •
monitoring will begin immediately. Upon submission of a valid test, this monthly test requirement will revert to
quarterly in the months specified above.
Should any test data from this monitoring requirement or tests performed by the North Carolina Division of Water
Quality indicate potential impacts to the receiving stream, this permit may be re-opened and modified to include
alternate monitoring requirements or limits.
If the Permittee monitors any pollutant more frequently than required by this permit, the results of such
monitoring shall be included in the calculation & reporting of the data submitted on the DMR&all AT Forms
submitted.
NOTE: Failure to achieve test conditions as specified in the cited document,such as minimum control
organism survival, minimum control organism reproduction,and appropriate environmental controls,
shall constitute an invalid test and will require immediate follow-up testing to be completed no later than
the last day of the month following the month of the initial monitoring.

Enclosed please find the original plus two copies of the following:
A. Check Number 5924 in the amount of$260.00 made payable to NC DENR.
B. The originally submitted application package minus the copy of the NPDES Permit.
As we discussed by phone, we were previously advised by phone that no permit fee would be
required. We have lost two plus weeks. In practical terms, this permit will result in no realized additional
flow to the stream. Since August of 2005, the existing facility has had no discharge. The capability to
discharge is required by NC Public Water Supply. It is noted that the existing permit has no flow limit,
and it is anticipated that there will be no flow limit associated with the revised permit. Since no"flow"
increase is represented in the effluent page, we request that you consider using the shortest available
permitting process. We have three USDA Funded water related projects that are awaiting issuance of this
permit.

Dear Mrs. Sprinkle:
This letter will serve as Southeastern Wayne Sanitary District's request for minor modification of
our NPDES Permit NC0086941. A speculative limits letter was issued on February 12,2010,which
supports doubling the size of the existing potable water treatment plant and its associated discharge. The
additional effluent page,which reflects this expansion,must be received prior to the District
receiving critical funding for three water related projects.
Southeastern Wayne Sanitary District will be increasing the size of the potable water treatment
plant from 0.9 MGD to 1.62 MGD. The quantity of wastewater will increase from 33,000 gpd to 66,000
gpd as a result of the expansion.The proposed water treatment plant expansion involves mirroring the
existing facilities on-site to include:three new iron sand filters,equal in capacity to the existing three iron
sand filters;filter backwash; ammonia addition at the wells for chloramine disinfection;two waste tanks
for solids settling,blending of decanted water with raw water coming into the plant; and dechlorination
with calcium thiosulfate,should the facility need to discharge. Discharge from the mirrored portion of the
facility will join with the existing facility, and utilize the existing discharge pipe.
As a reminder,there has been no discharge from this facility since August 2005. In lieu of
discharge,the water will continue to be blended with raw water coming into the plant. However,the
District must maintain the ability to discharge. Please find attached the following items:
1. A copy of the February 12,2010, speculative limits letter
2. A copy of the NPDES Permit No.NC0086941, issued on June 28,2011
3. Flow Schematic,and
4. Residuals Management Plan
On or about October 22,2008,this office requested to be decertified, since we were not
discharging. There is an agreement with the City of Goldsboro to analyze any samples until we are
recertified,in the event we need to resume discharging.

2008. Attached is a copy of the chemical analysis of the residuals.
