Rd. Widening. WBS No. 40255.3.3

Dear Mr. Lane,

Please find the enclosed revised permit drawing No. 19 of 22 and bore profile for a minor modification
request for the directional bore of (3) 1 ¼” HDPE conduit for Segra’s telecommunications line
relocation associated with the U-4906 Gum Branch Road widening project. The proposed total bore
length is 2306 linear ft. (~ STA 60+00 - ~STA 82+00) and the total bore diameter proposed is 6” to fit
the (3) HDPE conduit. The proposed bore pits are located outside of the Public Trust AEC in high
ground. As proposed, the bore depth will be approximately 10’ below the bottom substrate of Half Moon
Creek. No impacts to jurisdictional waters or wetlands are proposed by this bore. Erosion control
measures will be implemented in accordance with NCDOT’s Erosion and Sediment Control Plan and a
utility pre-construction meeting will be held to ensure NCDOT construction staff and the contractor are
prepared to respond to any issues including potential frac-outs of drilling slurry.

We anticipate $100.00 will be charged to the subject WBS to cover the processing fee for this minor
modification request. Please let me know if I can provide any additional information.
