Division of Water Resources' Central and Regional staff has reviewed the application package
received December 9, 2022. However, additional information is required before the review may be
completed. Please address the items on the attached pages no later than the close of business on March 10,
2023.
Please be aware that you are responsible for meeting all requirements set forth in North Carolina
rules and regulations. Any oversights that occurred in the review of the subject application package are
still the Applicant's responsibility. In addition, any omissions made in responding to the outstanding items
in Sections A through M, or failure to provide the additional information on or before the above requested
date may result in your application being returned as incomplete pursuant to 15A NCAC 02T .0107(e)(2).
Please reference the subject application number when providing the requested information. All
revised and/or additional documentation shall be signed, sealed and dated (where needed), with an

1. Per [15A NCAC 02T .0105(b)], adequate information regarding the proposed system and its
operation shall be provided. The LASC 06-16 form submitted states that Site JS-4 and JS-3 have
net areas of 8.66 and 7.27 acres, respectively. The Soils and Agronomy Report, however states that
JS-4 and JS-3 have net areas of 7.27 and 8.66 acres respectively. Please confirm which areas
correspond to Field JS-4 and JS-3 and confirm all other information regarding the fields given in
the LASC form are also correct.

J. Soils and Agronomy Report:
1. Per [15A NCAC 02T .0 1 05(c)(5)], location maps for sites shall be included. Within the Soils and
Agronomy Report for Jonathan Smith Farm, two maps are given. On the Soils Map, the application
area for JS-2 is split into an eastern and wester portion. On the Buffer Map, however, the western
portion of JS-2 has been greyed out. Additionally, the soil boring location performed in field JS-2
differs between the two maps.
Please clarify where land application will be occurring on field JS-2 and where the soil boring was
performed.
