Division of Water Resources’ Central and Regional staff has reviewed the application package
received February 3, 2023. However, additional information is required before the review may be
completed. Please address the items on the attached pages no later than the close of business on April 24,
2023.
Please be aware that you are responsible for meeting all requirements set forth in North Carolina
rules and regulations. Any oversights that occurred in the review of the subject application package are
still the Applicant’s responsibility. In addition, any omissions made in responding to the outstanding items

A. Overall:
1. Our office has reviewed the two existing permits pertaining to this minor modification request,
Holly Ridge Wastewater Treatment Facility (WQ0019907) and Summerhouse WWTP
(WQ0029945). A major modification has been submitted to our office for the new treatment
facility at Holly Ridge, and the proposed treated effluent design characteristics in that submittal
were taken into consideration. The proposed major modifications for WQ0019907 include a new
treatment facility designed to achieve effluent limits of 7 mg/L of total nitrogen and 3 mg/L of total
phosphorous to meet the groundwater lowering setback requirement of 100 feet in accordance with
15A NCAC 02T .0706(b). The Summerhouse WWTP permit has an effluent limit of 2 mg/L of
total phosphorus. This limit was established in the initial permit issuance dated May 23, 2006.
Please provide information explaining how the effluent limit of 2 mg/L of total phosphorus at the
existing Summerhouse infiltration sites will be met for treated effluent transferred from the new
treatment facility at Holly Ridge.

D. Application (Form: WWIS 06-16):
1. The response to Item IV.4 indicates that there is an attachment that lists applicable
permits/certifications to the proposed modification at this facility. This attachment did not appear
to be included in the submittal, and it is unclear whether any additional permits are necessary for
the proposed construction activities.
E. Property Ownership Documentation:
1. Please confirm that the entirety of the proposed alignment is either within the right-of-way or on
property owned by ONWASA (and the Town of Holly Ridge) and that no additional easements are
required to allow ONWASA access for construction and maintenance activities.
F. Engineering Plans:
1. The profile views on Plan Sheet C-2.07 and C-2.08 have labels for a 16-inch PVC force main. Our
understanding is that the proposed alignment for the effluent transfer main is to be constructed with
8-inch diameter piping. Please review and revise as necessary.
2. Sheet C-4.20 depicts a section of effluent transfer main that connects between the force main along
Holly Ridge Road and the infiltration pump station. Is this portion of the effluent transfer main
included in the 7,400 LF of 8-inch piping listed within the Project Narrative? The current plan and

profile views depict Sta. 10+00 to 65+30, and it is unclear where the remaining piping is to be
installed.
3. Within the plan view for the Pump Station effluent main on Sheet C-4.20, it appears that the force
main is to connect between the pump station and the effluent transfer main along Holly Ridge Road.
However, per Sheet C-2.06, an 8-inch temporary plug is proposed at Sta. 10+00. Please clearly
depict a plug/stub-out on the effluent main to be installed within the pump station area.
4. Detail drawings were not provided. Please provide detail drawings for trenching/pipe installation,
boring/jacking installation (please include any “spider” pipe supports and excavation/shoring
requirements for the bore/jack pit), minimum horizontal and vertical separation distances for
utilities, and appurtenances such as air release valves, valve boxes, temporary plugs, and manholes.
G. Specifications:
1. Specifications for Boring/Jacking and Horizontal Directional Drilling installation methods were
not provided. Please provide specifications for all proposed installation methods.
2. The provided leakage formulae on page 02530-15 do not appear to be in accordance with the most
recent revisions of AWWA C600/C605 that are referenced. Please review the referenced AWWA
standards and revise, if necessary.
