Guidelines for Access: The Zaff of the Regional Office is dedicated to making public records in our
custody readily available to the public for review and copying. We also have the responsibility to the public
to safeguard these records and to carry out our day-to-day program obligations. Please read carefully the
following before signing the form.
I. Due to the large public demand for file access, we request that you call at least a day in
advance to schedule an appointment for file review so you can be accommodated.
Appointments are scheduled between 9 00 a.m. and 3 00 p.m. Viewing time ends at 4:45
p.m. Anyone arriving without an appointment may view the files to the extent that time and
staff supervision are available.
2. You must specify files you want to review by facility name or incident number, as
appropriate. The number of files that you may review at one appointment will be limited to
five.
3. You may make copies of a file when the copier is not in use by the staff and if time permits.
There is no charge for 25 or less copies • cost per copy after the initial 25 is 2.5 per copy.
Payment is to be made by check, money order, or cash in the administrative offices.
4. Files must be kept in the order you received them. Files may not be taken from the office.
No briefcases large totes etc are permitted in the file review area. To remove, alter,
deface, mutilate, or destroy material in public files is a misdemeanor for which you can be
fined up to $500.00.
5. In accordance with GS 25-3-512, a $25.00 processing fee will be charged and collected for
checks on which payment has been refused.
& The customer must present a photo ID, sign -in, and receive a visitor sticker prior to
reviewing files.

Onslow County
A compliance inspection for Northwest Business Park was conducted on March 13, 2001.
At this time, the site was not in compliance. We received a March 22, 2001, letter from your
office requesting an extension to comply with the issues. At this time, we have not received any
information that the issues have been resolved. The outstanding issues are as follows:
Both ponds need pennanent vegetated cover established.
2. The orifice for Pond A may be clogged. The water level in the outlet ditch was
higher than the pond's water level.
Please let me know as soon as possible if these issues have been addressed. An applicant
is awaiting his permit based on the compliance of this system. If you have any questions
concerning this matter please feel free to call me at [phone].

#B". Let me know a confirm date that these discrepancies will be repaired for final

I have been contacted by the N.C. Department of Environment and Natural Resources
concerning stormwater issues at the Northwest Business Park project. I have enclosed a
copy of this letter for your use.
It is required by the State that any time you disturb any land of more than one (1) acre you
must be in compliance with Sedimentation & Erosion Control and Stormwater. As you
can see under item #3-C and #4-C, you have never submitted to the State for stormwater
approval.
You will need to contact the State for instructions to comply with the stormwater
regulations or you may contact me should you have any questions or need any assistance
in this matter. It is very important that you take care of this matter as soon as possible for
the State will only allow a certain amount of time to comply.

Due to the unstable weather at this time we would like to request for an extension on the

We did not do the design for the "Just Sofas" site but we are in the process of trying to
obtain the site plan to forward to you. Any consideration you could give us concerning

report summarizes the findings of a recent inspection of the project's stormwater controls conducted on March 13,
2001 to determine compliance with Stormwater Permit Number SW8 970517 issued on June 26,1997.
A summary of the findings and comments are noted under Section 4 of the report. Please inform this Office
in writing before April 13, 2001 of the specific actions that will be undertaken and the time frame required to correct
the deficiencies. Failure to provide the requested information, when required, may initiate enforcement action. If

a. Both ponds need permanent vegetated cover established.
b. The orifice for Pond A may be clogged. The water level in the outlet ditch was higher than the pond's
water level.
C. .lust Sofas has developed on Lot 5 and no plans were submitted to us for approval.
d. The restrictions provided to the Division on April 24, 2000, do not address all four of the required
statements, as found on Page 4 of the permit.
Required Actions:
Please repair eroded areas and establish a vigorous growth of permanent grass.
Please check the orifice and clear if needed. If the water level in the ditch is the culprit, please
investigate and unblock the ditch.
Please provide a set of the grading and layout plans for .lust Sofas.
Please amend the restrictions to include the other 3 statements and submit a recorded copy to the
Division.
