The Wilmington Regional Office received and accepted a State Stormwater Management Permit Renewal
Application for the subject project on June 23rd, 2022. A preliminary in-depth review of that information has
determined that the application is not complete and that additional information is needed. To continue the review,
please provide the following additional information in a single, hard copy submittal package:

15A NCAC 0211.1045(3)(11): Please include the following:
(i) Section 13.1 —Current Permit Holder's Company Name / Organization
(ii) Section 14.2 — Signing Official's Name
(iii) Section 13.3 — Signing Official's Title
(iv) Section BA — Mailing Address
(v) Section B.5 — Street Address
(vi) Section 13.6 — Phone & email
2. 15A NCAC 02H.1045(3)(e): Permittees are required to a have a copy of the agreed upon Operation and
Maintenance agreement, per the stormwater rules. Please indicate in Section CA whether or not you have
it copy of the Operation and Maintenance agreement on hand.
Please remember to confirm that any revised information is presented consistently throughout the application
documents. Please also remember to provide a hard copy of any updated documents per 15A NCAC
02H.1045(3).
The requested information should be received in this Office by July 15", 2022 or the application will be returned
as incomplete. if additional time is needed to submit the requested information, please email your extension
request prior to this due date to the email address provided below with ajustification and a date of when the
requested information will be submitted, which shall be no later than 15 days past the original due date. Please
note that only two letters requesting additional information is allowed in the State Stormwater Program for a
total of 30 days to submit the requested additional information. If the information is not satisfactorily provided
after either the second request or 30 days, the project will be returned. If the project is returned and you wish to
re -submit later, you will need to resubmit all required items at that time, including the application fee.

State Stormwater Management Permit #S W8 140701 for the subject project expires on February 2, 2023. This is
a reminder that permit renewal applications are due 180 days prior to their expiration per 15A NCAC 2H.1045(3).
North Carolina General Statutes and the Coastal Stormwater rules require that this property be covered under a
stormwater permit. Failure to maintain a permit subjects the owner to assessment of civil penalties. More
information about the Post -Construction state stomtwater program can be found on the following website:

Please submit a complete permit renewal form along with a $505.00 fee and other submittal requirements within
30 days. The permit renewal form can be found under the Post -Construction section of this website or at:

construction-2. Also, it was noted in the file review that a designer's certification that the project was constructed
in accordance with the approved plans as required by the permit has not been submitted. Please include this with

point of contact. Please submit a completed Permit Information Update Form to correct your contact information
which is available on the website provided above under the "New Permits & Permit Modifications" section.
