Dear Sir
As you are aware, Sections A & B of this project are currently under construction and the C
section is scheduled for Let on November 17, 2009 The current permits are set to expire on
December 31, 2008, however construction is not expected to be complete until December 31,
2010 Therefore, the Department would like to request a permmt extension to coincide with the
anticipated completion date '
No other changes to the existing permits are being requested at this time The Department will
subnut appropriate permit modification requests for Section C when final plans are complete
REGULATORY APPROVALS
Section 404 Permit The NCDOT requests that the referenced Individual Permit be extended as
described in this letter
Section 401 Permit We also are hereby requesting an extension to the 401 Individual
Certification and In compliance with Section 143-215 3D (e) of the NCAC, we will provide
$570 to act as payment for processing the Section 401 permit We are providing five copies of
