Division personnel have reviewed and approved your application for renewal of the subject permit.
Accordingly, we are forwarding the attached NPDES discharge permit. This permit is issued pursuant to
the requirements of North Carolina General Statute 143-215.1 and the Memorandum of Agreement
between North Carolina and the U.S. Environmental Protection Agency dated October 15, 2007 (or as
subsequently amended).

This final permit includes two major changes from the draft permit send to you on April 9,
1) A Total Monthly Flow (TMF) calculation and A. (2) Section are no longer necessary. The
TMF value and A. (2) Section were needed to calculate the TN Load which is no longer required.
2) A TN Load calculation, Monthly and Annually, has been removed from this permit. The
Division has determined that the TN Load impact from this facility does not warrant further
evaluation at this time. As nutrient conditions or other factors change in the Neuse River Basin
the Division will re-evaluate this permit requirements.
If any parts, measurement frequencies or sampling requirements contained in this permit are
unacceptable to you, you have the right to an adjudicatory hearing upon written request within thirty (30)
days following receipt of this letter. This request must be in the form of a written petition, conforming to
Chapter 150B of the North Carolina General Statutes, and filed with the Office of Administrative Hearings
(6714 Mail Service Center, Raleigh, North Carolina 27699-6714). Unless such demand is made, this
decision shall be final and binding.
Please note that this permit is not transferable except after notice to the Division. The Division
may require modification or revocation and reissuance of the permit. This permit does not affect the legal
requirements to obtain other permits which may be required by the Division of Water Quality or permits
required by the Division of Land Resources, the Coastal Area Management Act or any other Federal or
Local governmental permit that may be required.

to receiving waters designated as an unnamed tributary to Basal Creek in the Neuse
River Basin in accordance with effluent limitations, monitoring requirements, and
other conditions set forth in Parts I, II, III and IV hereof.
This permit shall become effective July 1, 2008.
This permit and authorization to discharge shall expire at midnight on April 30, 2013.

All previous NPDES Permits issued to this facility, whether for operation or discharge are hereby revoked As of this
permit issuance, any previously issued permit bearing this number is no longer effective. Therefore, the exclusive
authority to operate and discharge from this facility arises under the permit conditions, requirements, terms, and

Aqua North Carolina, Inc. is hereby authorized to:
1. Continue to operate a well -water treatment system that discharges wastewater

1. Daily flow estimates are determined by the number and duration of backwash/rinse events.
2. Total Residual Chlorine compliance is required for chlorine or chlorine derivative used for disinfection. The
facility shall report all effluent TRC values reported by a NC certified laboratory including field certified.
However, effluent values below 50 µg/L will be treated as zero for compliance purposes.
3. The pH shall not be less than 6.0 s.u. or greater than 9.0 s.u..
4. For a given wastewater sample, TN - (NO2-N + NO3-N) + TKN, where TN is Total Nitrogen, (NO2-N + NO3-N) is
Nitrite/Nitrate Nitrogen, and TKN is Total Kjeldahl Nitrogen.
All samples collected should be from a representative discharge event.

A NPDES WASTEWATER PERMIT
On the basis of thorough staff review and application of NC
General Statute 143,215.1 and 15A NCAC 02H.0109 and
other lawful standards and regulations, the North Caroli-
na Environmental Management Commission proposes to
issue a National Pollutant Discharge Elimination System
(NPDES) wastewater discharge permit to the person(s)
listed below effective 45 days from the publish date of this
notice.
,hritten comments regarding the proposed permit will be
accepted until 30 days after the publish date of this notice.
All comments received prior to that date are considered in
the final determinations regarding the proposed permit.
The Director of the NC Division of Water Quality may
decide to hold a public meeting forthe proposed permit,
should the Division receive a significant degree of public
interest.
Copies of the draft permit and other supporting informa-
tion on file used to determine conditions present in the
draft permit are available upon request and payment of
the costs of reproduction. Mail comments and/or requests

Town of Hookerton, NC0025712, has applied to renew its
NPDES permit discharging to Contentnea Creek in the
Neuse River Basin. Total Residual chlorine (TRC) and
fecal coliform ore water -quality limited. This discharge
may affect future wasteload allocations to the receiving..
BP Products North America (7401 Old Mt. Holly Road,
Charlotte, North Carolina 28214) has applied for renewal of
NPDES permit NCO036145 for the Selma Terminal at 3707
Buffalo Road in Selma, North Carolina. This permitted
facility discharges wastewater to on unnamedtributgryto
Mill Creek in the tJeuse River Basin. Currently, total
suspended solids and phenol are water quality limited.
Thisdischarge may affect future allocations in this portion
of,Mill Creek.
Jerry G. Williams & Sons, Inc. (P.O. Box 2430, Smithfield,
NC 27577) has applied for renewal of permit NCO085936 for
the company's Wet Deck Log Storage site In Johnston
County. This permitted facility discharges stormwater
and wet -decking 'runoff to an unnamed tributary to the
Neuse River in the Neuse River Basin. Currently, no
parameters are water -quality limited. This discharge
may affect future allocations in this portion of the Neuse
River.

General:
The renewal application was returned to the consultant, return #2270, on December 27, 2007. There
were several missing items. A follow up letter was also send to Aqua North Carolina, Inc. stating the
application could not be process until a correct application was provide.
The correct application was received January 15, 2008. A fast track dechlorination AtC was issued in
March 2005. Aqua North Carolina, Inc. became the new owner in 2007. The application stated the
permitted flow monthly average would be 0.00171 MGD.
Effluent Limited criteria are:
- Flow, monthly average max, monitor to determine TN Load
- TSS, daily max, monthly average max
- TN, calculate allocated N lbs
- (NO2-N + NO3-N), monitor to determine TN
- TKN, monitor to determine TN
- TP, monitor only
- Total Iron, monitor
Water Quality Limited criteria is:
- pH, daily range limit
- TRC, daily max.
The 2008 renewal application included a copy of the Sludge Management Plan.
Process:
Three parallel sand filter provide water treatment for the well water. Chlorination is done upstream
of the filters. The sand filters backwash and other waste water are discharged through a tablet
dechlorinator and then to a sand filter bed were the effleunt continues to the outfall. The accumulated
sludge in the sand filter is removed by a contractor as needed. March 2005 AtC given for tablet
dechlorination treatment which is added to the waste water stream prior to entering the final sand
filter. Discharge is to UT to Basal Creek.
Numerous paperwork violations.
Concerns:
Flow effluent monitoring/reporting, per last permit it is estimated based on backwash events. DMR
data review indicates monthly average flow would exceed the 0.00171 MGD monthly average so the
permitted value was increased to 0.0025(BPJ).

TSS monitoring with Effluent Daily and Monthly Average Limits. Sample is grab. Will increase
frequency to 2/month with previous and limits. DMR data review indicates no violations, daily value
typically less than 5 mg/L.
Settleable Solids with Effluent Daily and Monthly Average Limits. Sample is grab. One value at
Daily Max. Will not continue per latest guidelines.
Turbidity monitoring with Effluent Daily Limit. Sample is grab. No violations. Turbidity typically
less than 10 ntu. Will not continue per latest guidelines.
Iron monitoring, currently monitoring only. Sample is grab. Typically less than 5 mg/L. Will
increase frequency to monthly.
TRC monitoring with Water Quality Daily Limit. Sample is grab. Will need to modify footnote
to state definition of chlorine additives, compliance date already enforced. Prior to compliance date
had values above limit but installed dechlorination. Will increase frequency to 2/month.
.pH monitoring with Water Quality Daily Limits. Requires grab sample. New requirement per
latest guidelines. Frequency is 2/month.
Total Nitrogen (TN) monitoring, currently monitoring only with measurements of TKN and (NO2-
N + NO3-N). TN Load calculations and reports submitted monthly and annually. Sample is grab.
Will maintain current frequency. Per DMR data TN Load annually is less than 3 lbs/yr.
Total Phosphorus (TP) monitoring, currently monitoring only. Sample is grab. Will maintain
current frequency. Based on DMR data TP less than 1 mg/L.
Changes to renewal permit:
1. Continue all monitoring parameters and limits for TN, TN Load, TP as in existing modified
permit.
2. Edit permit, owner name to reflect Aqua, updated component list.
3. Remove Settleable Solids and Turbidity.
4. Edit TRC existing footnote to show only definition for chlorine additive, increase frequency.
5. Edit Iron monitoring to increase frequency.
6. Edit TSS to increase frequency.
7. Add pH with limits.
Rational For Changes:
This WTP facility is a small discharger. The historical data indicates very small TN Load and TP.
Does not have allocation or limit therefore will continue frequencies.
Per guidelines need to add pH with limits and Total Manganese monitoring. All limited parameters
need to be measured 2/month while monitored parameters of significance should be measured
monthy.
TRC footnotes need to clearly define the additives. The limit is enforced and no longer requires
compliance time line.
REVISIONS NOTES. 411108: Fluoride, Zinc, and Manganese are NOT required since no
chemical additions are part of the treatment

Facility Name: Stansted Well #2 WTP
The sludge removal consists of passing filter backwash water through sand filter bed which in
turn goes to effluent stream discharge. Sludge management for this facility consists of the
raking of sludge from the 400 sf sand filter and hauled to a landfill by Aqua North Carolina, Inc.
The removal service occurs regularly on an as needed basis as determined by Aqua North
Carolina, Inc. operating personnel.
Sludge Collection and Disposal shall conform to 40 CFR 503 of the Clean Water Act.
Every shipment of sludge will be recorded in a log book. This log book will provide a record of:
1. date of shipment
2. volume removed off site
3. destination of sludge
Sludge Collection and Disposal Tracking:
