This letter is in response to the Notice of Violation, Tracking # NOV-2019-DV-0445, concerning the spill'
at 5875 Riverdale Rd. Outfall, Guilford County, High Point, NC.
The spill at the above -mentioned address lasted for 3 hours. During the spill duration crews worked to
remove the top section of a flat top manhole which required special equipment to lift due to its weight.
Maintenance personnel had to be called in to turn off the lift station. The flat top needed to be removed
so the crew could shut down the discharging air release valve. It was determined that the air release
had mechanically failed causing the valve seat to fall through the body and jam itself in the butterfly
(isolation) valve. Once the air release was removed the isolation valve was closed, the lift station was
restarted, and the spill stopped. The crew installed a backup air release valve while onsite. All solids
were removed, and lime was spread over the area. The creek was monitored for adverse effects. The
air release that failed has been rebuilt and is ready to be installed should it be needed. The air release
has been added to a weekly inspection list to prevent future spills.
