Division personnel have reviewed and approved your permit renewal application. Accordingly, we are
forwarding the attached NPDES discharge permit. This permit is issued pursuant to the requirements of North
Carolina General Statute 143-215.1 and the Memorandum of Agreement between North Carolina and the U.S.
Environmental Protection Agency dated May 9, 1994 (or as subsequently amended).
If any parts, measurement frequencies or sampling requirements contained in this permit are unacceptable to
you, you have the right to an adjudicatory hearing upon written request within thirty (30) days following receipt of
this letter. This request must be in the form of a written petition, conforming to Chapter 150B of the North
Carolina General Statutes, and filed with the Office of Administrative Hearings (6714 Mail Service Center, Raleigh,
North Carolina 27699-6714). Unless such demand is made, this decision shall be final and binding.
Please note that this permit is not transferable except after notice to the Division. The Division may require
modification or revocation and reissuance of the permit. This permit does not affect the legal requirements to obtain
other permits which may be required by the Division of Water Quality or permits required by the Division of Land
Resources, the Coastal Area Management Act or any other Federal or Local governmental permit that may be
required.

MGD (see Part I. A. (3.)).
3. Discharge from said treatment works at the location specified on the attached
map into the Catawba River, classified WS-IV & B CA waters in the Catawba
River Basin.

A. (3.) PHASED CONSTRUCTION REQUIREMENTS
If this facility is to be expanded in phases, plans and specifications for the next phase of
expansion shall be submitted when the flow to the existing system reaches 80% (0.064
MGD) of the design capacity of the installed components (0.08 MGD).
At no time may the flow tributary to the facility exceed the design capacity of the installed
components.

5. Directions to Site: From the jct. of Hwy. 115 and SR 1102 (Langtree Rd.) in the
community of Mount Mourne, travel southwest on SR 1102 Z 1.6 miles and turn left on
Castaway Trail (No SR Number). The WWTP site is at the end of Castaway Trail
(beyond the cul-de-sac) on a gravel access road.

C. Describe receiving stream features and pertinent downstream uses: The point of
discharge is in an area immediately adjacent to a large arm of Lake Norman. The
area adjacent to the point of discharge receives frequent bodily contact from a
variety of primary recreational activities including, but not limited to: swimming,
skiing, wading, etc,. There is a water intake for the Town of Davidson located
1.0 mile towards the main channel of the Lake.
PART Il - DESCRIPTION OF DISCHARGE AND TREATMENT WORKS

Description of existing or substantially constructed WWT facilities: The existing
WWT facilities consist of an influent flow equalization tank followed by a bar
screen, a flow splitter box, dual aeration tanks (diffused), dual clarifiers, a sludge
holding tank (aerated), dual tertiary filters, dual tablet disinfection units, an
effluent flow recorder with totalizer, post aeration, effluent pumps, and a stand-by
power generator.
f. Description of proposed WWT facilities: There are no proposed WWT facilities.
g. Possible toxic impacts to surface waters: Chlorine is added to the waste stream.
h. Pretreatment Program (POTWs only): Not Needed.

PART IV - EVALUATION AND RECOMMENDATIONS
The permittee, Mid -South Water Systems, requests renewal of the subject Permit. With
the exception of minor structural improvements, there have been no changes and/or
modifications to the WWTP since the Permit was last issued.
Such being the case, this Office recommends that the Permit be renewed as requested
provided no negative comments are received from DEH and pending receipt and approval of the

of operation by Heater Utilities, Inc. began on or about 6/17/99.

11/99, monthly flows averaged 0.04646 MGD. No limit violations were reported during
review period. Last inspection was conducted on 1/6/00.

Prior to 8/10/99 Mid South Water Systems, Inc. was permittee. Actual purchase and start
of operation by Heater Utilities, Inc. began on or about 6/17/99.

sludge will be removed by contract septage haulers and disposed of in various
