Dear Mr. Weber:
A letter of request for an authorization to Construct was received
April 9, 1986 by the Division and final plans and specifications for the subject
project have been reviewed and found to be satisfactory. Authorization is hereby
granted for the construction of a 0.010 NM wastewater treatment facility consisting
of a 1,575 gallon capacity aerated flow equalization tank with dual 10 GPM pumps and
high-water alarm; a flow splatter box; dual 5,000 gallon capacity aeration tanks, each
with an influent bar screen; dual 65 CFM blowers; an 1,820 gallon capacity sludge
holding tank; a 1,667 gallon capacity clarifier, a 200 gallon capacity baffled
chlorine contact tank with 4-tube tablet chlorinator; an 810 gallon capacity
reaeratlon tank with air diffuser, dual 10 GPM effluent pumps, and high-water alarm; a
recording effluent flow meter; approximately 600 lineal feet of 2-inch effluent force
main with diffuser for submerged stream discharge; on -site generator for emergency
standy by paver; and all associated piping, valves, fittings and appurtenances to
serve Castaway Shores Subdivision.
This Authorization to Construct is issued in accordance with Part III
paragraph C of the NPDES Permit No. ECD0561-54 issued May 19, 1986, and shall be
subject to revocation unless the wastewater treatment facilities are constructed in
accordance with the conditions and li=itations specified in Permit No. 9=56154.
The Permittee must employ a certified wastewater operator in
accordance with Part III, paragraph D of the reference permit.
The sludge generated from these treatment facilities'*ust be disposed
of in accordance with G.S. 143-215.1 and in a manner approvable by the Ncrth Carolina
Division of Environmental Yanagement.

The Mooresville Regional Office, telephone number 704/663-1699 shall
be notified at least twenty-four (24) hours in advance of startup of the installed
facilities so that an in -place inspection can be made of said facilities prior to
operation. Such notification to the Regional Supervisor shall be made during the
normal office hours from 8:00 a.m. until 5:00 p.m. on Monday through Friday, excluding
State Holidays.
In event the facilities fail to perform satisfactorily in meeting
its NPDES pertit effluent limits, the perwittee shall take such immediate corrective
action as say be required by this Division, including the construction of additional
wastewater treatment and disposal facilities.
