The project involved the discharge of fill material into South Deep Creek associated with the construction of a dam and subsequent flooding of Deep Creek and several of its unnamed tributaries, for the purpose of constructing a municipal /industrial water supply and flood control reservoir, known as 5D. Specifically, the project is located off SR 1314, approximately 4 miles west of Yadkinville, Yadkin County, North Carolina. Reference is also made to the site inspections of April 23, 2014, and November 24, 2014, by me with your representative, Mr. Jason Walker to conduct final inspections of the work associated with the referenced DA permit and the mitigation sites associated with the project. Based on the monitoring reports and our site inspections over the past five years, we have determined that the site has met the required hydrologic and vegetative success criteria for both the stream and wetland mitigation components of your mitigation plan. We have determined that you have satisfied the mitigation requirements for Yadkin County's Deep Creek Watershed Structure 5D Mitigation Project (Dinkins Bottom Wetland Mitigation Site, Carl Shore Stream Restoration and Effie G. West Stream Enhancement), and we concur with closing out the monitoring requirements associated with your permit. In addition, we have determined that the project has been constructed according to the plans is in compliance with our permit issued on July 22, 2008.
