Mill Creek is located within common areas of the Village Walk Condominiums. The stream has been subject to fairly extensive erosion due to past stream armoring activities. The
surrounding areas are residential.

To enhance the hydrologic function of Mill Creek to reduce erosion and encourage proper sediment transport and deposition.

The proposed enhancement efforts are concentrated on a 980 foot stream reach of Mill Creek. The project is to be constructed utilizing small to mid size machinery. It is anticipated that a
12,000 - 18,000 lb track hoe along with a 9,000 to 12,000lb skid steer will be utilized to complete the project. During structure installation a pump around set up will be used.

GRADING DRAINAGE AND EROSION CONTROL NOTES
1. GRADING CONTRACTOR SHALL MEET WITH CREEKSTONE, PLLC (CS, PLLC) ON -SITE TO REVIEW CLEARING
DEMOLITION AND GRADING PRIOR TO STARTING WORK.
2. EXCAVATED MATERIAL THAT IS SUITABLE AS DETERMINED BY A GEOLOGIST, GEO-TECHNICAL AND/OR
STRUCTURAL ENGINEER MAYBE USED ON SITE AS DIRECTED BY CS, PLLC.
3. WORK LIKELY TO DISLOCATE, DAMAGE OR IMPAIR THE STRENGTH OF EXISTING UTILITIES OR STRUCTURES
SHALL BE DONE ONLY AFTER ADEQUATE PROTECTION HAS BEEN PROVIDED. THE CONTRACTOR SHALL
IMMEDIATELY REPAIR AND PAY FOR ALL DAMAGES.
4. ALL FINISHED GRADES SHALL BE WITHIN A'/2 " TOLERANCE OF GRADES SHOWN ON THE PLAN.CS, PLLC

PIPES SHALL BE BURIED 24" AND EXTENDED PER THE PLAN OR AS DIRECTED BY CS, PLLC. PIPE SHALL BE
6" CORRUGATED PLASTIC PIPE UNLESS OTHERWISE NOTED.
8. EROSION CONTROL MEASURES ARE GENERAL IN NATURE. CONTRACTOR IS RESPONSIBLE FOR
ADDITIONAL MEASURES AS REQUIRED TO PREVENT ON OR OFF SITE RUNOFF AND EROSION.
9. ALL INLETS TO HAVE TEMPORARY INLET PROTECTION INSTALLED IMMEDIATELY AFTER INLET HAS BEEN
INSTALLED.
10. CONTRACTOR IS RESPONSIBLE FOR WEEKLY INSPECTION OF ALL EROSION AND SEDIMENT CONTROLS
AND INSPECTION OF ALL EROSION AND SEDIMENT CONTROLS AFTER EACH RAIN EVENT. PERFORM ANY
REQUIRED MAINTENANCE TO ENSURE OPTIMUM CONTROL OPERATION.
11. REMOVE ALL TEMPORARY EROSION AND SEDIMENT CONTROLS AFTER DISTURBED AREAS HAVE BEEN
STABILIZED AND REVIEWING GOVERNMENT AGENCY HAS APPROVED REMOVAL.
12. ALL TREES, UNDERSTORY AND OTHER VEGETATION TO REMAIN SHALL BE PROTECTED FROM INJURY
DURING ANY LAND CLEARING AND CONSTRUCTION ACTIVITY.
13. CONTRACTOR SHALL NOT PARK VEHICLES, STORE MATERIALS OR TRENCH WITHIN THE DRIPLINE OF
TREES OR OTHER VEGETATED AREAS TO REMAIN.
14. CONTRACTOR SHALL NOT ALLOW CLEARING OF EQUIPMENT, STORAGE OR DISPOSAL OF MATERIALS
SUCH AS CONSTRUCTION MATERIALS, PAINT, SOLVENTS, ASPHALT OR CONCRETE WITHIN VEGETATED
AREAS TO REMAIN OR ANYWHERE ON SITE.
15. MATERIALS AND COMPACTION UNDER THE BUILDING SHALL CONFORM TO THE PLANS AND
RECOMMENDATIONS OF A GEOLOGIST, GEO-TECHNICAL AND/OR STRUCTURAL ENGINEER.
16. NOTICE TO CONTRACTOR: THE STARTING OF ANY WORK ON THE SITE ESTABLISHES THAT THE
CONTRACTOR WITHOUT RESERVATION ACCEPTS THE CONTRACT DOCUMENTS AS ACCURATELY
REPRESENTING THE EXISTING CONDITIONS.
17. SEEDING AND MULCHING OF DISTURBED AREAS SHALL TAKE PLACE WITHIN 14 CALENDAR DAYS OF ANY
PHASE OF GRADING. ON SLOPES STEEPER THAN 3:1 AND SLOPES LONGER THAN 50, SHALL BE STABILIZED
WITHIN 7 DAYS IN ACCORDANCE TO NCG01 SECTION IIB.
18. UNLESS OTHERWISE STATED, ALL FILL AREAS SHALL BE CONSTRUCTED IN LAYERS OF 8- MAXIMUM
THICKNESS WITH WATER ADDED OR SOIL CONDITIONED TO THE OPTIMUM MOISTURE CONTENT AS
DETERMINED BY A GEOTECHNICAL AND/OR STRUCTURAL ENGINEER AND COMPACTED WITH A SHEEP'S
FOOT ROLLER TO A COMPACTION EQUAL TO OR GREATER THAN 95% (100% IN THE TOP 2 ' OF THE
SUBGRADE BELOW ROADWAYS AND PARKING LOTS) OF THE DENSITY OBTAINED BY COMPACTING A
SAMPLE OF THE MATERIAL IN ACCORDANCE WITH THE STANDARD PROCTOR METHOD OF
MOISTURE -DENSITY RELATIONSHIP TEST, ASTM D698 OR AASHTO-99 UNLESS SPECIFIED IN OTHER
SPECIFICATIONS.
19. ENTIRE AREA TO BE GRADED SHALL BE CLEARED AND GRUBBED. NO FILL SHALL BE PLACED ON ANY
AREA NOT CLEARED OR GRUBBED.
20. ALL SOIL EROSION CONTROL MEASURES REQUIRED BY THE GRADING PLAN SHALL BE PERFORMED
PRIOR TO GRADING, CLEARING, OR GRUBBING. ALL EROSION CONTROL DEVICES SUCH AS SILT FENCE
TEMPORARY SEDIMENT TRAPS, ETC., SHALL BE MAINTAINED IN WORKABLE CONDITION FOR THE LIFE OF THE
PROJECT AND SHALL BE REMOVED AT THE COMPLETION OF THE PROJECT ONLY AT THE CS, PLLC'S
APPROVAL OR THE SIGN OFF OF THE JURISDICTIONAL EROSION CONTROL OFFICE. ALL EROSION CONTROL
DEVICES SHALL BE INSPECTED AFTER ANY PRECIPITATION EVENT EXCEEDING 1/2", AND IN NO INSTANCE
LONGER THAN 1 WEEK. PAYMENT SHALL BE CONSIDERED INCIDENTAL TO CLEARING AND GRUBBING. IF
DURING THE LIFE OF THE PROJECT A STORM CAUSES SOIL EROSION WHICH CHANGES THE FINISH GRADES
OR CRATES GULLIES OR WASHED OUT AREAS THE CONTRACTOR SHALL REPAIR AT NO ADDITIONAL CHARGE.
SILT WASHED OFF -SITE ONTO ADJACENT PROPERTY SHALL BE REMOVED AS DIRECTED BY CS, PLLC AT NO
EXTRA COST. THE CONTRACTOR SHALL ADHERE TO ANY APPROVED EROSION CONTROL PLANS WHETHER

1. SET UP PUMP WITH SUCTION AND DISCHARGE HOSE.
2. INSTALL UP -STREAM SANDBAG DAM.
3. INSTALL DOWN -STREAM SANDBAG DAM.
4. THE PUMP MUST RUN CONTINUOUSLY WHILE WORKING IN THE STREAM.
5. STREAMBANKS MUST BE STABILIZED AT THE END OF EACH DAY.
NOTES:
1. SANDBAG DIKES SHALL BE SITUATED AT THE UPSTREAM AND DOWNSTREAM ENDS OF THE WORK AREA, AND STREAM FLOW
SHALL BE PUMPED AROUND THE WORK AREA. THE PUMP SHOULD DISCHARGE ONTO A STABLE VELOCITY DISSIPATER
CONSTRUCTED OF RIPRAP OR SANDBAGS.
2. WATER FROM THE WORK AREA SHALL BE PUMPED TO A SEDIMENT FILTERING MEASURE SUCH AS A SEDIMENT BAG OR OTHER
APPROVED DEVICE. THE MEASURE SHALL BE LOCATED SUCH THAT THE WATER DRAINS BACK INTO THE CHANNEL BELOW THE
DOWNSTREAM SANDBAG DIKE WITHOUT CAUSING FURTHER EROSION BETWEEN THE SEDIMENT FILTER BAG AND THE STREAMBANK.
