This partnit, issued on 1129L15 __ ___ , is subject to compliance with the application (where consistent
with the permit), all applicable regulations, special conditions and notes set forth below. Any violation of these terms may
be subject to fines, imprisonment or civil action; or may cause the permit to be nu11 and void.
Road Shoulder/Shoreline Stabiliza�ion and Gravel Turn Around
1) In order to protect anadromous fisheries resources in the Cashie River, no in-water vvork shaIi be
conducted between February 1 Sth and June 3pth of any year without prior approval of the N.C.
Division af Coastal Managennent (DCM}, in consultation with the U.S. Army Corps of Engineers.
2) The alignment of the authorized revetments shall be staked by the penmittee and approved by a
representative of DCM within a maximum of 30 days prior to the start of any construction. Fa.ilure to
initiate construction within 30 days, or erosion of the shoreline by adverse weather canditions shall
require the alignment to be restaked by the permittee and re-approved by DCM within a maxirnum of
30 days prior to the new expectec� start of construction.

This permit action may be appealed by the permittee or
o�her qualified persons within twenty (20) days of the issuing
date.
T'his pertnit must be accessible pn-site to Department
personnel when the project is inspected for compliance.
Any maintenance work or project modi�cation not covered
hereunder requires further Di�ision approval.
All work must cease when the permit expires on
December 31, 20�8
In issuing this permit, the State of North Carolina agrees
that your project is consistent with #he North Carolina Coasta]
Management Program.

ADDITIONAL CONDITIONS
3) The authorized revetment material shall not extend waterward more than 8 feet fram the normal water
level at any point along its alignment at the tizne of construction, without permit modificatian.
4) Na fill material shall be placed at any time in any vegetated wetlands or surrounding waters outside of
the alignment of ihe fill axeas as iz�dicated an the attached workplan drawings, without permit
modification.
S) All fill material shall be clean and free of any pol�utants except in trace quantities.
6} Filter fabric shall be in place priar to the placement of any re�etment material.
7) Placement of revetment material and fill for the gravel t�rnaround shall be �imited to th� areas as
depicted on the attached workplan drawings. This rr�aterial shall be free frain loose dirt or any pollutant
except in trace quantities.
8) The temporary placement or double handling of fill materials within waters ar vegetated wetlands is no#
authorized.
General
NOTE: This praject vaill permanently impact appro�imately �30 square feet of 404 wetlands and
approximately 732 square feet of surface waters.
9) This project shall conform to all requirements of the N.C. Sedimentation Pollution Control Act and the
N.C. Department of Transportation's (NCDOT's) Memorandum of Agreement with the N.C. Division of
Energy, MineraI, and Land Resources.
10) If it is determined that additional permanent andlor texnporary impacts are necessary that are not shown
on the attached workplan drawings ar described in the authorized permit application, a permit
rnodification and/or additional authorization from DCM may be required. In addition, any changes in
the approved plan may also require a permit modification and/or additional autharizatian from DCM.
The permitt�e shall contact a representative of DCM prior to commencement of any such activity for
this determinatian and any permit modification.
11} The permittee shall exercise all available precautions in the day-to-day operations ofthe facility to
prevent waste from entering the adjacent waters and wetlands.
12) No attempt shall be made by the permittee to prevent the full and free use by the public of all navigabie
waters at or ac�jacent to the authorized work following completion of construction and demolition
activities.

The permittee andlor his contractor shall contact the DCM Transportation Pra�ect Field Representative
in Elizabeth City at {252} 264-3901 to request a preconstruction conference prior to project initiation.

NOTE:
The N.C. Divisian of Water Resources (DWR) authorized the proposed project (DWR Project
No. 14-1325) under Genera� Water Quality Certiiication No. 390�.
The U.S. Army Corps of Engineers autharized the proposed project under Regional General
Permit Number 198Q00291 (COE Action ID No. SAW-2015-00047), which was issued on
119/15. '
This permit does not eliminate the need to obtain any additional state, federal or local permits,
approvals or authorizations that may be required.
