Pin #s 2587739509 and 2587913666 Neuse River Basin Wayne County, North Carolina Gentlemen: As authorized, Mr. Benjamin V. Wilson, P.E. of Cardno ATC visited the site in November and December 2014 and observed the entire site and the low lying areas shown on the attached maps. The property consists of two parcels totaling 113.93 acres The properties with Wayne County Co. Pin #s 2587739509 and 2587913666 are owned by STONEMOR NORTH CAROLINA LLC. Maps of the properties are included in the appendix and photos are included with the wetlands and stream assessment forms in the Appendix. A preliminary determination was made of the streams and wetlands in accordance with the following methods: Wetlands for 404 Permitting - A determination was made of the wetlands based on the Corps of Engineers (COE) 1987 Wetlands Manual using the routine method and the Regional Supplement to the Corps of Engineers Wetland Delineation Manual: Atlantic and Gulf Coastal Plain Region (Version 2.0) November 2010 which is the currently accepted methodology. Streams for 404 Permitting by the (COE) - A preliminary determination was made of the streams based on our experience with the most recent methodology used by the US Army Corp of Engineers. Wetlands and Stream Determination — Wayne Memorial Gardens 113.93 Acres Wayne County, North Carolina — December 16, 2014 • Streams for 401 Permitting and Determination of Stream Buffers by the NCDENR Division of Water Quality - A determination was made of the streams using the North Carolina Division of Water Quality (NCDWQ) stream determination form method. The site is located in the Poplar Branch watershed in the Neuse River Basin. The site is located on Poplar Branch in the Neuse River Basin. The Neuse River is approximately 1 mile downstream from the site. The site is shown as having one stream (Poplar Branch) on the Wayne County Soil Survey and one on the Southwest Goldsboro USGS map (see attached maps). A summary of features at the site is included in the tables below: Wetlands and Streams Subject to 404 Permitting by COE if Impacted Feature Amount (Approximate) Description Notes Wetlands 1.93 Acres None Neuse River Rules Streams - Perennial 352 + /- linearft Flow year round 50 ft Buffer each side Streams Intermittent 0 +/- linear ft Only flow for part of Neuse River Rules the year 50 ft Buffer each side Only flow during These may be Streams - Ephemeral 0 +/- linear rainfall and dry up mentioned on the quickly after event. 404 permit but are 'Riparian Buffer under the Neuse River Rules and others requires a 50 ft buffer on each side of the stream. Maps with approximate locations of the site and site features are enclosed with this letter. 2 Wetlands and Stream Determination — Wayne Memorial Gardens 113.93 Acres Wayne County, North Carolina — December 16, 2014 However, a field visit may be required by both NCDENR DWQ and USACOE. Prior to the site visits an agent authorization form will be required. We are in the process of scheduling a site visit by the appropriate personnel. An agent authorization form should be signed and emailed or faxed to me to be turned in with the site layout at the appropriate time.
