Route 4, Box 390 New Bern, North Carolina 28560 SUBJECT: Permit No. NCO056618 Reissue of Authorization to Construct Dated July 19, 1984 Carolina Pines Country Club, Inc. Wastewater Treatment Facility - Craven County Dear Mr. Vernelson: The Division of Environmental Management on December 7, 1984, received modified plans and specifications for the subject project and your letter of request for an amended Authorization to Construct. The final changes have been reviewed and found to be satisfactory. The original Authorization to Construct, dated ,duly 19, 1984, is hereby MGD rescinded and authorization is granted for the construction of a 0._125 aerated wastewater treatment facility consisting of a 4000 gallon capacity aerated surge tank with dual submersible pumps and high-water alarm; bar screen; inflow splitter box; dual wastewater treatment lines, each with a 62,500 gallon capacity aeration tank, 6283 gallon capacity sludge holding tank and 9750 gallon capacity clarifier; two 350 CFM blowers with a stand-by unit; 2605 gallon capacity chlorine contact tank with three sanuril tablet -type chlorinators; a 4000 gallon capacity effluent dosing tank with dual submersible pumps and high-water alarm; stand-by power generation equipment; and all associated s, including approximately 5,800 lineal feet of piping, valves and appurtenance 6-inch effluent discharge force main with. an. affluent diffuser located 750 feet off shore, to serve the Carolina Pines Development. This Authorization to Construct is issued in accordance with Part III paragraph C of NPDES Permit No. NCO056618 issued November 1, 1983, and shall be subject to revocation unless the wastewater treatment facility are constructed in accordance with the conditions and limitations specified in Permit No. NC0056618. The Permittee must employ a certified wastewater operator in accordance with Part III paragraph D of the referenced permit. Cont'd. =anai Ov^orrunm• Affirmative Ac:,on=:nDiovcr Permit No. NCO056618 Page Two The sludge generated from these treatment facilities must be disposed of in accordance with G.S. 143-215.1 and in a manner approvable by the North Carolina Division of Environmental Management. In event the facilities fail to perform satisfactorily in meeting it's NPDES permit effluent limits, the permittee shall take such -immediate corrective action as may be required by this Division, including the construction of additional wastewater treatment and disposal facilities. The effluent diffuser shall be installed -no -closer than 750 feet off shore in approximately 4.8 feet of water at mean low water for adequate effluent dissipation.
