Does the submitted system comply with 15A NCAC 02T, the Minimum Design Criteria for the Permitting of Pump Stations and Force Mains (latest version), and the Gravity Sewer Minimum Design Criteria (latest version) as applicable? ® Yes ❑ No If no, for projects requiring a single variance, complete and submit the Variance/Alternative Design Request application (VADC 10-14) and supporting documents for review to the Central Office. Approval of the request will be issued concurrently with the approval of the permit, and projects requiring a variance approval may be subject to longer review times. For projects requiring two or more variances or where the variance is determined by the Division to be a significant portion of the proiect, the full technical review is required. 2. Professional Engineer's Certification: I, Benjamin B. Thomas, attest that this application for the 2022 Water & Sewer Systems Rehabilitation Project, SRP-W-0185 (Professional Engineer's name from Application Item III.1.) (Project Name from Application Item II.1) has been reviewed by me and is accurate, complete and consistent with the information supplied in the plans, specifications, engineering calculations, and all other supporting documentation to the best of my knowledge. I further attest that to the best of my knowledge the proposed design has been prepared in accordance with the applicable regulations, Minimum Design Criteria for Gravity Sewers (latest version), and the Minimum Design Criteria for the Fast -Track Permitting of Pump Stations and Force Mains (latest version). Although other professionals may have developed certain portions of this submittal package, inclusion of these materials under my signature and seal signifies that I have reviewed this material and have judged it to be consistent with the proposed design. NOTE — In accordance with General Statutes 143-215.6A and 143-215.613, any person who knowingly makes any false statement, representation, or certification in any application package shall be guilty of a Class 2 misdemeanor, which may include a fine not to exceed $10,000, as well as civil penalties up to $25,000 per violation. Misrepresentation of the application information, including failure to disclose any design non-compliance with the applicable Rules and design criteria, may subject the North Carolina -licensed Professional Engineer to referral to the licensing board. The project location is within the Town of Beech Mountain in Avery and Watauga Counties. The project purpose is to replace aging and failing infrastructure, as well as some orangeburg piping on Skiway Road. The project will not serve any new customers or have any additional flow. Thomas, PE bthomas@west-consultants.com PROJECT DESCRIPTION: Town of Beech Mountain 2022 Water & Sewer Systems Rehabilitation Project [.Y1142MAVU-31. The project is located in the Town of Beech Mountain in Avery and Watauga Counties. The primary purpose of the project is to replace existing gravity sewers in two locations in the south part of Beech Mountain. These are the Ski Loft Road and Skiway Road (USGS & Street Level Maps No. 1), which are aging and failing infrastructure. The estimated length of pipe to be replaced is 1,717'. The other location is the Skiway Road area (USGS & Street Level Maps No. 1), which contains orangeburg pipe. Additionally, it is unknown exactly how properties are connected to the collection system or where the sewers are located. The known sewers are below standards, i.e., no manholes for many segments, under -sized, or unknown material. The length in this area is 4,735'. There is also 440' of 8" pipe and 671' of 10" pipe proposed to be replaced in the middle and northern part of Beech Mountain (USGS & Street Level Maps No. 2). All proposed gravity sewer pipe will be SDR 26 PVC or ductile iron pipe. Since no new flow is being added, a USE form was not completed.
