Enclosed is a copy of the Compliance Evaluation Inspection for the inspection conducted at the subject
facility on January 25, 2023, by Ori Tuvia. Your cooperation during the site visit was much appreciated. Per our
discussion, please see attached permit name/Ownership Change Form (mark it has other and explain).
As discussed on site, the following items needs to be addressed:
1. The facility has failed to collect chlorine samples since 2019, the facility must sample semi-annually.
2. Facility must calculate the flow estimated to be discharged, it is recommended the facility either use
the change of depth or the pump run time to estimate the flow.
3. Internal sampling for pH should be taken on the same day that flow was discharged.
4. All records should include the dates, times and name(s) of individual(s) performing the sampling.
