facility on January 25, 2023, by Ori Tuvia. Your cooperation during the site visit was much appreciated. Please
advise the staff involved with this NPDES Permit by forwarding a copy of the enclosed repot. Per our discussion
please see attached Permit Name/Ownershp Change Form (mark it as other and explain).
The site review conducted on January 25, 2023 revealed the site has been connected to the city of Mount
Holly sewer system and is not discharging to the creek, therefore the permit may be rescinded. If interested in
rescinding the permit, please submit a written request to:
