Please check your records and determine if the invoices were paid. If the invoices have been paid, please forward
documentation of payment to my email address below.
If the invoices have not been paid, please remit payment as soon as possible to the address indicated on the invoice and
notify me when a check has been cut. There are no late fees associated with the subject invoices. The next annual
permit fee will be sent out in October 2014.

Thanks for returning my call to discuss the subject overdue annual permit fees.
We agreed upon a payment plan of one month for the outstanding annual permit fees.
Terms: A check/money order for the first of two payments in the amount of$860.00 must be received by our Division
no later than 5/15/14. The second check/money order $860.00 must be received by our Division no later than
6/2/14. Failure to pay either of the overdue fees by the dates agreed upon will result in referral to the AGs Office for
collection.
