Dear Mr. & Mrs. Council,
Ron Boone, Seren Homer, Jesse Barnes, and Kristin Potwora, of the NC Division of Water
Resources (DWR), Winston-Salem Regional Office (WSRO), met with you, current owner of 2701
Lockwood Drive in Winston-Salem, NC, to perform a compliance evaluation inspection of the
home's single family wastewater treatment system, on February 8, 2023. The inspection findings
are summarized below and in the attached checklist.
• You are the current owner of the residence and the current permittee.
• In accordance with the NCG550000 permit, you must have the septic tank pumped out
completely at least every 5 years, or when the solids level in the tank reaches 1/3 or more
of the liquid depth of the tank. You are aware of the location of the septc tank and stated
that you do have it pumped out regularly. There were no obvious signs of tank malfunction
or problems. You should keep all receipts/invoices of when you have the tank pumped as
a record of it. Keep the records in a file with all other related records, such as analytical
results, annual fee invoices, etc.
• There were no signs of malfunction or problems with the subsurface sand filter, however,
the filter's exact location is unknown.
• There were tablets in the chlorinator at the time of the inspection and you are using the
proper tablets (i.e. main ingredient is calcium hypochlorite). You should probably put no
more than five tablets at a time in the chlorinator. Once you better determine an accurate
usage rate, you should place no more tablets in the tube than would be used up over a
month's time. Calcium hypochlorite is a very potent oxidizer and can cause severe irritation
of skin, eyes, etc. Be sure to wear eye, respiratory, and hand protection when handling the
tablets. Do NOT breathe fumes from the tablets. Thoroughly rinse any body parts that
contact the tablets with fresh water.

• The outfall pipe into the stream was located. It is readily accessible, and no problems or
issues were noted.
• No laboratory analyses of the effluent have been completed yet. We provided you a copy
of the general permit and went through its primary requirements with you. You may find
certified laboratories capable of running the required tests for you at this website:

We believe the Pace Labs office in Kernersville is the closest one to your residence.
• As you may already know, there is a $60 annual fee for this permit. It will be sent to you

nothing at this time. You will simply receive a copy of the renewed permit once it is
completed.

Dear Mr. & Mrs. Council,

(DWR), Winston-Salem Regional Office (WSRO), met with you, current owner of 2701 Lockwood
Drive in
Winston-Salem, NC, to perform a compliance evaluation inspection of the home's single family
wastewater
treatment system, on February 8, 2023. The inspection findings are summarized below and in the

- You are the current owner of the residence and the current permittee.
- In accordance with the NCG550000 permit, you must have the septic tank pumped out completely at
least every 5 years, or when the solids level in the tank reaches 1 /3 or more of the liquid depth of the
tank. You are aware of the location of the septc tank and stated that you do have it pumped out
regularly. There were no obvious signs of tank malfunction or problems. You should keep all
receipts/invoices of when you have the tank pumped as a record of it. Keep the records in a file with
all other related records, such as analytical results, annual fee invoices, etc.

- There were tablets in the chlorinator at the time of the inspection and you are using the proper
tablets (i.e. main ingredient is calcium hypochlorite). You should probably put no more than five
