Dear Ms. Gill:
Ron Boone, of the NC Division of Water Resources (DWR), Winston-Salem Regional Office
(WSRO), came to your home at 5864 Whitney Rd in Graham, NC, to perform a compliance
evaluation inspection of the home's single family wastewater treatment system, on February 15,
2023. No one answered the door that day so Mr. Boone could not perform the inspection.
Additionally, we could not reach you by phone or email because we have no contact information
for you on file.
Please contact Mr. Boone as soon as possible to schedule a firm date to perform an inspection to
ensure your wastewater treatment system is operating properly. Failure of the system can result in
costly repairs, environmental damages, damages to your home, and health hazards in the home.
Furthermore, please be aware that the Division may take enforcement action against any permittee
who fails to comply with any requirement of their permit. Pursuant to NC General Statute (NCGS)
143-215.6A, a civil penalty of not more than twenty-five thousand dollars ($25,000.00) may be
assessed against any person who violates or fails to act in accordance with the terms, conditions,
or requirements of any permit issued pursuant to G.S. 143-215.1.
