REQUEST FOR MORE INFORMATION
Dear Mrs. Flowers:
On August 7, 2007, the Division of Water Quality (DWQ) received your application dated August 7,
2007, with additional information received September 4, 2007 and September 14, 2007, to impact 4,890
square feet (ft2) of Zone 1 protected riparian buffers and 4,464 square feet (ftZ) of Zone 2 protected
riparian buffers to construct the proposed bridge at the above subject property. The DWQ has determined
that your application was incomplete and/or provided inaccurate information as discussed below. The
DWQ will require additional information in order to process your application to impact protected
wetland, streams and/or buffers on the subject property. Therefore, unless we receive the additional
information requested below, we will have to move toward denial of your application as required by 15A
NCAC 2H .0506 and 2B .0233(8) and will place this project on hold as incomplete until we receive this
additional information. Please provide the following information so that we may continue to review your
project.
Additional Information Requested:
1. Please provide a separate detailed profile of your proposed bridge.
2. Please revise your plan sheet to show where the bents are located. If there are not bents, be sure to
clearly illustrate the type of approach you are using.
3. On your revised plan sheet, please clearly mark the stream.
4. Please explain why the bridge cannot span Zone 1 of the protected riparian buffers. It appears that
the bridge could span Zone 1 to avoid and minimize the proposed impacts to Zone 1 of the
protected buffers.
5. Please show on your revised plan sheet how the banks under the bridge will be stabilized.
6. Clearly show on your revised plan sheet where the end bents are for the bridge in reference to the
streambank. ore

Please respond within three weeks of the date of this letter by sending this information to me in writing
and Lauren Witherspoon of the DWQ Raleigh Regional Office. If we do not hear from you within three
weeks, we will assume that you no longer want to pursue this project and we will consider the project as
withdrawn.
This letter only addresses the application review and does not authorize any impacts to wetlands, waters
or protected buffers. Please be aware that any impacts requested within your application are not
authorized (at this time) by the DWQ. Please call Cyndi Karoly or Amy Chapm at 919-733-1786 if
