This letter is in response to the Notice of Violation NOV-2021-DV-0234 that the City of High
Point received dated May 3, 2021 for illegal discharge of Sodium Hypochlorite. The Eastside
W WTP followed all reporting requirements upon the discovery of the discharge of the Sodium
Hypochlorite into the polishing filters. On April 20, 2021, the date that the incident was
discovered, John Thomas, Eastside W WTP ORC was unable to reach Mr. Ron Boone at the
office or his cell phone. Mr. Thomas then called the Emergency Management number to
verbally report the incident. No instructions for additional monitoring or sampling were given
by Emergency Management during that call. The next day, April 21, 2021, Mr. Thomas received
a call from Mr. Boone to follow tip on the initial report. Mr. Boone then gave additional
monitoring instructions that were immediately followed by the Eastside W WTP.
The City of High Point's response to the incident were as follows:
1. The operator who was responsible for the incident was responded to in accordance with
the City's Personnel Resolution.
2. All Eastside W WTP personnel have been re-trained in the areas of bulk chemical storage
operations, chemical feed systems operations, and spill and emergency response.
