Long Term Monitoring Plan, Headworks Analysis (including allocation table) and Industrial Waste Survey were submitted to the NPDES Municipal Permitting Unit- Pretreatment office on 9/28/2022 as required by our NPDES #NC0021211 permit which had an effective date of 4/1/2022. Our office has not received approval of these items as of today, 9/28/2022. • Cintas Corporation, Metal Impact East, Novaflex Hose Inc. and Stericycle, Inc. permits were all renewed July 1, 2021 and will expire June 30, 2026. • Prescient Company Inc.'s(IUP #3001) was issued a new permit effective May 2, 2022 with an expiration date of May 1, 2027. Our office received notice that Prescient had not been running their paint line since 4/15/2022. On 5/9/2022 we received notice from Prescient that they had closed and would have no more process discharge. They were in the process of moving some of their equipment to other locations, selling other equipment and closing down the facility to vacate. We notified the NPDES Municipal Permitting Unit- Pretreatment office of this information as part of our updating the IWS in September, 2022. We received the official drop permit notification from the NPDES Municipal Permitting Unit- Pretreatment office on 11/7/2022 with request to forward the final inspection of Prescient when this was done. We were able to do the final inspection on 11/28/2022 and forwarded this information as requested to the NPDES Municipal Permitting Unit - Pretreatment office. We have removed Prescient from our allocation table. Our pretreatment program had added USA Dutch, Inc., a 433 categorical industry, which we permitted during 2021, but due to construction delays and delivery delays of their new paint line, they did not begin discharge until 2/15/2022. They have been complying with all permit requirements. They did complete their TOMP in May, 2022 and are now certifying for their TTO requirements, therefore there are no results for the second 6- month period (7/1/2022—12/31/2022) for these parameters. There permit will expire in 12/11/2026. City of Graham had no industries in Significant Noncompliance in 2022. Data on the Division's Pretreatment Program Information Database was all correct, with the exception that after dropping Prescient's IUP #3001, we now have 5 SIU's and 3 CIU's. Also, I attended the PAR workshop virtually on 2/9/2022. Historical SNC Report was received from the Municipal Permitting Unit — Pretreatment office for our facility indicating Novaflex, IUP#1001, as having historical SNC for BOD. The time frame for their SNC was not marked, but we indicated the time frame with a red X. It was during the July 1, 2021 — December 31, 2021 time period. Included is the older allocation table which shows Prescient Company, Inc. since they were part of our program for part of 2022. Also included is the updated allocation table with Prescient Company removed. General Permit Information: • SIU Permits for Cintas Corporation, Metal Impact East, Novaflex Hose, Inc., Stericycle, Inc. and USA Dutch, Inc. are now in their second year of the five-year permit cycle. There have been no permit modifications for these industries to date. SIU Permit for Prescient Company, Inc. has been dropped due to company closure. SIU Information Cintas Corporation - Permit No. 0007 (Industrial Laundry) 610 Woody Drive, Graham, N.C. 27253 Cintas Corporation had the following activities during the Jan.l- Jun. 30, 2022 reporting period: • Cintas Corporation had one BOD violation this reporting period for which a Notice of Non -Compliance was issued. The Notice of Non -Compliance required 1 additional resample. This resample was taken by Cintas Corporation for the parameter and the resample was within their permit limit. They were not in SNC for BOD. • Cintas Corporation had one TSS violation this reporting period for which a Notice of Non -Compliance was issued. The Notice of Non -Compliance required 2 additional resamples. These resamples were taken by Cintas Corporation for the parameter and the resamples were within their permit limit. They were not in SNC for TSS. • Orders and Schedule Information: None • A to C and Construction Information: None Cintas Corporation had the following activities during the Jul. I- Dec.31, 2022 reporting period: • Cintas Corporation had one BOD violation this reporting period for which a Notice of Non -Compliance was issued. Metal Impact East Permit No. 200140CFR 467.36 (Aluminum Forming) 235 Riverbend Rd., Graham, N.C. 27253 Metal Impact East had the following activities during the Jan. 1-Jun. 30, 2022 reporting period: • Metal Impact East had no violations this reporting period to report. • Metal Impact East was monitored for TTO compounds under 40 CFR 467. 36. Due to a high level or organic acids, esters and other non -target compounds present in the sample the laboratory was unable to achieve lower detection limits for semi -volatile and volatile organic analysis. This sample required a x10 dilution. All values were not detected. • Orders and Schedule Information: None • A to C and Construction Information: None Metal Impact East had the following activities during the Jul. I- Dec. 31, 2022 reporting period: • Metal Impact East had no violations this reporting period to report. • Metal Impact East was monitored for TTO compounds under 40 CFR 467. 36. Prescient Company, Inc. Permit No. 300140 CFR 433.17 (Metal Finisher) 2125 Senator Ralph Scott Parkway, Mebane, NC 27302 Prescient Company, Inc. had the following activities during the Jan. 1- Jun. 30, 2022 reporting period: • Prescient Company, Inc. had no violations this reporting period to report. • Orders and Schedule Information: None • A to C and Construction Information: None • Total Toxic Organics Certification Statement received 6/10/22. • Prescient informed us by letter that they would be ending production and thus ending discharge from the facility/paint line on 5/9/22. They had not had discharge from the paint line since 4/15/22. • Process of dropping the industry began in May, 2022. They were planning to sell their equipment; therefore, a final inspection was delayed until they were prepared to move out the equipment. Prescient Company, Inc. had the following activities during the July 1-Dec. 31, 2022 reporting period: • No discharge from Prescient since 4/15/22, therefore no sampling was done during the 7/1/2022—12/31/2022 6-month period. The facility had closed as of 5/9/22 and began the process of moving out and selling equipment. • Notice was sent to the NPDES Municipal Permitting Unit — Pretreatment in September, 2022 with our updated IWS with explanation of the procedure of moving and selling equipment that Prescient was undergoing. • Our office received the approved drop request for Prescient's Permit 93001 on 11/7/2022 from the NPDES Municipal Permitting Unit — Pretreatment office. • Final inspection of the Prescient facility was sent to the NPDES Municipal Permitting Unit — Pretreatment office on 2/3/22. The final inspection at Prescient was done 11/28/22. 5. Stericycle, Inc. Permit No. 0030 (Medical Waste) 1168 Porter Ave., Haw River, N.C. 27258 Due to the extremely high levels of matrix interferences present in the Stericycle samples during samplings events during 2022, the commercial lab which was used for metals was unable to reach the lower detection limits for metals that are stated in the City of Graham LIMP during some sampling events. The commercial lab did provide documentation as to the cause of this. This documentation is kept with the sample results and is available for review if needed. Stericycle had the following activities during the Jan. 1- Jun. 30, 2022 reporting period: • Stericycle had no violations this reporting period to report. • Orders and Schedules Information: None • A to C and Construction Information: None Stericycle had the following activities during the July 1- Dec. 31, 2022 reporting period: Stericycle had one Cadmium violation this reporting period for which a Notice of Non -Compliance was issued. The Notice of Non -Compliance required 3 additional resamples. Theses resamples were completed by Stericycle and the resamples were within their permit limit. They were not in SNC for Cadmium. 0 Stericycle had one TKN violation this reporting period for which a Notice of Non -Compliance was issued. Due to a high level or organic acids, esters and other non -target compounds present in the sample the laboratory was unable to achieve lower detection limits for volatile organic analysis and required a x10 dilution.
