received on November 29, 2022. However, additional information is required before the review may be
completed. Please address the items on the attached pages no later than the close of business on February
23, 2023.
Please be aware that you are responsible for meeting all requirements set forth in North Carolina
rules and regulations. Any oversights that occurred in the review of the subject application package are
still the Applicant's responsibility. In addition, any omissions made in responding to the outstanding items
in Sections A through F, or failure to provide the additional information on or before the above -requested

Our records indicate that Mr. Van Wagenberg is not a currently designated Back -Up ORC for this
facility. Ms. Aldridge is designated as the ORC but only Matthew Sechler is designated as the
Back -Up ORC. Please submit the attached Operator Designation Form with Mr. Van Wagenberg's
information on it. Please follow the directions at the bottom of the form as it should be sent to the
WPCSOCC and the Mooresville Regional Office. After this has been completed, Mr. Van
Wagenberg will be added as a Back -Up ORC in our records. [NCAC 15A 8G .0201]
C. Section IX— Irrigation Fields:
1. The Rowan County GIS portal showsthat parcels 265-001 and263-008 have been sold from Amber
Waves Holdings, LLC to Carolina Malt House, Inc. Please confirm this and update the "Deeded
Owner" column.
D. Attachment A — Site Map:
1. "Yes" was selected, but a site map was not provided. Please submit a site map pursuant to the
requirements in 15A NCAC 02T .0504(d).
E. Attachment L — Setback Waivers:
1. The existing permit does include setback waivers but "No" was selected. Pursuant to 15A NCAC
02T .0506(d), please provide these setback waivers that have been written, notarized, signed by all
parties involved, and recorded with the Rowan County Register of Deeds.
