the volume of wastewater to be permitted in this project has been evaluated along the route to the receiving
wastewater treatment facility and that the flow from this project is not anticipated to cause any capacity
related sanitary sewer overflows or overburden any downstream pump station en route to the receiving
treatment plant under normal circumstances, given the implementation of the planned improvements
identified in the planning assessment where applicable. This analysis has been performed in accordance
with local established policies and procedures using the best available data. This certification applies to
those items listed above in Sections I and II plus all attached planning assessment addendums for which I
am the responsible party. Signature of this form certifies that the receiving collection system or treatment
works has adequate capacity to transport and treat the proposed new wastewater.

Please find the following items enclosed for your review and approval:
1. One (1) original Fast -Track Sewer System Extension Application with supporting maps.
2. One (1) copy of a Fast -Track Sewer System Extension Application with supporting maps.
3. One (1) copy each of Environmental Permits/Authorizations.
4. Review fee check in the amount of $480.
This project includes a 535 linear feet public gravity sewer extension to serve the currently under
construction pretreatment plant upgrades located on the Grifols facility in Clayton, NC. Additionally, there
is a length of the gravity sewer line that will be privately owned. The proposed line was sized for a future
flow from the upgraded pretreatment plant, but any future flows not currently permitted from the Grifols
facility will be permitted accordingly at that time. The existing gravity sewer from Grifols facility outfalls to
an existing Johnston County pump station located on Grifols property. The current force main from the
County's pump station is routed to the Town of Clayton's ECIA pump station and then pumped into a force
main along US 70 Business Highway West. The proposed line is meant to bypass the existing county
pump station and flow directly to the ECIA pump station time as shown on the attached site maps without
adding additional flow at this. This bypass is to help facilitate a future decommissioning of the County's
pump station. Approximately 276 linear feet of the proposed line will be an aerial crossing with ductile iron
pipe and reinforced concrete piers located outside of the stream and wetlands. The gravity line falls under the
15A NCAC 02B .0233 protection rules and was approved by the NC DWR permitting branch on July 8, 2020.
SAW-2020-00915 falls under the 401 Water Quality Certifications and does not require a permit. It was
approved by the U.S. Army Corps of Engineers on June 6, 2020.

Location description: The project area is located at the existing Grifols Therapeutics facility, approximately 0.43 mile northeast
of the intersection of US Highway 70 Business and Powhatan Road, Just south of the existing railroad, in Clayton, Johnston
County, North Carolina.
Description of Activity: The Grifols/Clayton Waste Water Effluent Line project consists of —1,600 linear feet of new effluent
line connector from the existing Grifols facility on US 70 Hwy Bus West southeast to a new Town of Clayton pump station.
The effluent line bridees the wetland/stream complex (by several vertical feet) and has no pilings/foundations within the
wetland/stream area (see attached plan sheets S101, S201, S301, S21038, and S21039). Any tree clearing within the wetland
area required for installation/maintenance of the effluent line would be ahoveground (i.e. no grubbing) and accomplished
through hand clearine methods. Any trees cleared within the wetland boundary will be cut and lifted out of the wetland from
equipment based on high -ground to prevent eround disturbance within the wetland. Visibility fencing will also be installed
along/outside of the wetland boundary to prevent equipment from entering the wetland and to prevent incidental rutting
disturbance.
Your work as proposed does not require Department of the Army authorization for the following reason(s):
(J The entire property is composed of high ground. Therefore, your proposed activity will not occur in any aquatic resources.
O The proposed activity will occur in non jurisdictional waters andior wetlands.
® The proposed activity is not regulated and/or does not include regulated discharges of dredged or fill material.
A jurisdictional determination was not completed with this request. This letter does not address nor include any
consideration for jurisdiction on aquatic resources and shall not be interpreted as such. This is not an appealable
action. However, you may request an approved JD, which is an appealable action, by contacting the Corps for

Dear Mr. Russell:
You have our approval for the impacts listed below for the purpose described in your
application received by the Division of Water Resources (Division) on May 21, 2020 with
payment received on May 27, 2020. These impacts are covered by the attached Water Quality
General Certification Number 4132 and the conditions listed below. This certification is
associated with the use of Nationwide Permit Number 12 once it is issued to you by the U.S.
Army Corps of Engineers. Please note that you should get any other federal, state or local
permits before proceeding with your project, including those required by (but not limited to)
Sediment and Erosion Control, Non -Discharge, and Water Supply Watershed regulations.
This approval requires you to follow the conditions listed in the enclosed certification and the
following additional conditions:
1. The following impacts are hereby approved provided that all of the other specific and
general conditions of the Certification and Buffer Rules are met. No other impacts are

2. Diffuse Flow
All stormwater must be directed and maintained as diffuse flow at non -erosive velocities
through the protected stream buffers such that it will not re -concentrate before discharging
into a stream. [15A NCAC 02B .0233(8)]
3. The permittee shall report to the Raleigh Regional Office any noncompliance with this
certification, any violation of stream or wetland standards [15A NCAC 02B .0200] including
but not limited to sediment impacts, and any violation of state regulated riparian buffer
rules [15A NCAC 02B .0200]. Information shall be provided orally within 24 hours (or the
next business day if a weekend or holiday) from the time the applicant became aware of the
circumstances. A written submission shall also be provided within 5 business days of the
time the applicant becomes aware of the circumstances. The written submission shall
contain a description of the noncompliance, and its causes; the period of noncompliance,
including exact dates and times, if the noncompliance has not been corrected, the
anticipated time compliance is expected to continue; and steps taken or planned to reduce,
eliminate, and prevent reoccurrence of the noncompliance. The Division may waive the
written submission requirement on a case -by -case basis.
4. Protective Fencing
The outside buffer, wetland or water boundary and along the construction corridor within
these boundaries approved under this authorization shall be clearly marked with orange
warning fencing (or similar high visibility material) for the areas that have been approved to
infringe within the buffer, wetland or water prior to any land disturbing activities to ensure
compliance with 15 NCAC 02H .0500. [15A NCAC 02H .0506 (b)(2) and (c)(2) and 15A NCAC
02H .0507 (c)]
S. This approval is for the purpose and design described in your application. The plans and
specifications for this project are incorporated by reference as part of the Certification and

Buffer Authorization. If you change your project, you must notify the Division and you may
be required to submit a new application package with the appropriate fee. If the property is
sold, the new owner must be given a copy of this approval letter and General Certification
and is responsible for complying with all conditions. [15A NCAC 02B .0507(d)(2)]
6. This approval and its conditions are final and binding unless contested. [G.S. 143-215.5]
This Certification and Buffer Authorization can be contested as provided in General Statute
150E by filing a written petition for an administrative hearing to the Office of Administrative
Hearings (hereby known as OAH) within sixty (60) calendar days.
