In accordance with the application for a discharge permit received on February 23, 1996, the Division is
forwarding herewith the subject NPDES permit. This permit is issued pursuant to the requirements of North
Carolina General Statute 143-215.1 and the Memorandum of Agreement between North Carolina and the US
Environmental Protection Agency dated December 6,1983.
If any parts, measurement frequencies or sampling requirements contained in this permit are unacceptable
to you, you have the right to an adjudicatory hearing upon written request within thirty (30) days following
receipt of this letter. This request must be in the form of a written petition, conforming to Chapter 150B of the
North Carolina General Statutes, and filed with the Office of Administrative Hearings, Post Office Drawer
27447, Raleigh, North Carolina 27611-7447. Unless such demand is made, this decision shall be final and
binding.
Please take notice this permit is not transferable. Part II, EA. addresses the requirements to be followed
in case of change in ownership or control of this discharge.
This permit does not affect the legal requirements to obtain other permits which may be required by the
Division of Water Quality or permits required by the Division of Land Resources, Coastal Area Management
Act or any other Federal or Local governmental permit that may be required.

TO DISCHARGE WASTEWATER UNDER THE
NATIONAL POLLUTANT DISCHARGE ELIMINATION SYSTEM
In compliance with the provision of North Carolina General Statute 143-215.1,
other lawful standards and regulations promulgated and adopted by the North Carolina
Environmental Management Commission, and the Federal Water Pollution Control Act, as
amended,
Carolina Water Service, Inc. of N.C.
is hereby authorized to discharge wastewater from a facility located at
Abington Subdivision
Bainburgh Circle
north of Kernersville
Forsyth County
to receiving waters designated as Belews Creek in the Roanoke River Basin
in accordance with effluent limitations, monitoring requirements, and other conditions set forth in
Parts I, II, III, and IV hereof.
This permit shall become effective February 1, 1997
This permit and authorization to discharge shall expire at midnight on January 31, 2002

SUPPLEMENT TO PERMIT COVER SHEET
Carolina Water Service, Inc.
is hereby authorized to:
1. Continue to operate an existing 0.200 MGD wastewater treatment plant consisting of an
equalization basin with dual pumps, bar screen and flow splitter box, dual aeration basins, dual
clarifiers, tablet chlorinators, chlorine contact tank, dechlorination unit, post aeration chamber,
Stevens flow recorder and standby generator located at Abington Subdivision WWTP, Bainburgh
Circle, north of Kernersville, Forsyth County (See Part III of this Permit), and
2. After receiving an Authorization to Construct from the Division of Water Quality, construct
additional facilities not to exceed 0.385 MGD (See Part III of this permit), and
3 Discharge from said treatment works at the location specified on the attached map into Belews
Creek which is classified Class C waters in the Roanoke River Basin.

E. Phased Construction Condition
If this facility is built in phases, plans and specifications for the next phase shall be submitted when
the flow to the existing units reaches 80% of the design capacity of the facilities on line. At no time
may the flow tributary to the facility exceed the design capacity of the existing units.
PART IV
ANNUAL ADMINISTERING AND COMPLIANCE MONITORING FEE
REQUIREMENTS
A. The permittee must pay the annual administering and compliance monitoring fee
within 30 (thirty) days after being billed by the Division. Failure to pay the fee in a
timely manner in accordance with 15A NCAC 2H .0105(b)(4) may cause this Division to
initiate action to revoke the permit.

This is a permit renewal for a discharge into Belews Creek. This facility is in the process of expanding
to 0.2 MGD with an ultimate design flow of 0.385 MGD. Per Mike Mickey (W-SRO), permit should be
renewed without previous 0.1 MGD limits page. Within the past twenty four months this facility had
violations of BOD, TSS and Fecal Coliform. Since expansion to .2 MGD will be completed in the near
future, Permits and Engineering will recommend existing limits at expansion flows of .2 MGD and .385
MGD only with /monitoring reflective of a class II facility.
