Dear Mr. Barrier
A letter of request for Authorization to Construct was received April 2,
= 1985, by the Division and final plans and specifications for the subject project-
have been reviewed and found to be satisfactory. Authorization is hereby granted
for the construction of a 0.007 MGD wastewater treatment facility consisting
of a 7,000 gallon capacity aeration tank, a 1,.077 gallon capacity sludge holding
tank, a 1,511 gallon capacity clarifier, a 250 gallon capacity chlorine contact
tank with tablet type chlorinator, dual 41 CFM blowers, a 510 gallon capacity
tank used for both post aeration and residual chlorine removal requirements and
associated valves, piping and appurtences to: serve the Jonas Ridge Nursing Home.
This Authorization to Construct is issued in accordance with Part III paragraph
C of NPDES Permit No. NCO060224 issued November 30, 1984, and shall be subject
to revocation unless the wastewater treatment facilities are constructed in accordance
with the conditions and limitations specified in Permit No. NCO060224.
- The Permittee must employ a certified wastewater operator in accordance
with Part III paragraph D of the referenced permit.
The sludge generated from these treatment facilities must be disposed of
in accordance with G.S. 143-215.1 and in a manner approvable by the North
Carolina Division of Environmental Management.
The Mooresville Regional Office, telephone number 7041663-1699 shall be
notifed at least twenty-four (24) hours in advance so that an in-place inspection
can be made of said system prior to operation. Such notification to the Regional
Supervisor shall be made during the normal office hours from 8:00 A.M. until
5:00 P.M. on Monday through Friday, excluding State Holidays.

In event the facilities fail to perform satisfactorily in meeting NPDES
permit effluent limits, the Permittee shall take such immediate corrective
action as may be required by this Division, including the construction of
additional wastewater treatment and disposal facilities.
