In accordance with the application for a discharge permit received on November 9, 1998, the Division is
forwarding herewith the subject NPDES permit.. This permit is issued pursuant to the requirements of
North Carolina General Statute 143-215.1 and the Memorandum of Agreement between North Carolina
and the U.S. Environmental Protection Agency dated December 6, 1983, and as subsequently amended.
Please note the following changes from the previous permit (as indicated in the draft) have been finalized:
Total Residual Chlorine (TRC) monitoring 2/Week at outfall 002 has been added; however,
monitoring is required only if facility ever uses chlorine -for disinfection (monitoring frequency per
NCAC T15A: 2B .0500s). Also, upon expansion to 0,03 MGD, a TRC limit of 28 µg/1 will apply
(again, only if chlorine used). This change is consistenfwith current Division policy for new flows
(flow capacity not yet constructed).
Quarterly monitoring for total nitrogen and phosphorus has been added. This change is
consistent with an effort to gather more data about nutrient loading in this portion of the watershed.
Please also note one change from the draft in the final permit. Effluent Limitations and Monitoring
Requirements pages A(2). and A(3). incorrectly referred to outfall 001 in the draft permit an_ d have
both been modified to refer to outfall 002 instead.
If any parts, measurement frequencies, or sampling requirements contained in this permit are
unacceptable to you, you have the right to an adjudicatory hearing upon written request within thirty (30)
days following receipt of this letter. This request must be in the form of a written petition, conforming to
Chapter 150B of the North Carolina General Statutes, and filed with the Office of Administrative
Hearings, Post Office Drawer 27447, Raleigh, North Carolina 27611-7447. Unless such demand is made,
this decision shall be final and binding.

Please take note that this permit is not transferable. Part II, E. 4. addresses the requirements to be
followed in case of change in ownership or control of this discharge. This permit does not affect the legal
requirements to obtain other permits which may be required by the Division of Land Resources or any
other Federal or Local governmental permit that may be required.

SUPPLEMENT TO PERMIT COVER SHEET
Candle Corporation of America, Inc.
is hereby authorized to:
1. Continue to discharge non -contact cooling water from outfall 001, and
2. Operate two (2) existing 0.01 MGD wastewater treatment facilities, the first
consisting of a bar screen, aerated equalization basin, extended aeration, clarifier,
aerated sludge holding basin, chlorine contact tank, and a V-notch weir, and the
second consisting of an equalization basin, aeration chamber, clarifier, sludge holding
chamber, and a chlorine contact chamber, for a combined treatment capacity of 0.02
MGD of domestic wastewater discharged from outfall 002, located at Candle
Corporation of America, Inc., Gentry Lane, Elkin, Surry County, and
3. After receiving an Authorization to Construct (ATC) from the Division of Water
Quality, increase the plant capacity by adding an additional 0.010 MGD treatment
plant for an ultimate design capacity of 0.030 MGD, and
4. Discharge from said treatment works at the location specified on the attached map
into the Yadkin River, which is classified as C waters in the Yadkin -Pee Dee River
Basin.

The Permittee shall obtain authorization from the Division prior to the use of any chemical additive in the discharge. The Permittee
shall notify the Director in writing at least ninety (90) days prior to instituting use of any additional additive in the discharge which
may be toxic to aquatic life (other than additives previously approved by the Division). Such notification shall include completion of
Biocide Worksheet Form 101 (if applicable), a copy of the MSDS for the additive, and a map indicating the discharge point and
receiving stream.
There shall be no discharge of floating solids or visible foam in other than trace amounts.

review these documents very carefully to ensure thorough understanding of the information, conditions,
and requirements contained therein.
You should be aware that the following changes have been incorporated into your permit:

• Ambient station USGS 02112250 is nearby, but it is located just upstream of the discharge. This
segment of the Yadkin is Support Threatened and is affected by sediment. It received a Good -Fair
bioclassification in 1996.
• No particular strategies for this area of the watershed, aside from identifying specific sources of
sedimentation.

A . Biocide Condition
The ferrhi rite eWail not use any biocides except those approved in conjunction with the
c permit application. The permittee shall notify the Director in writing not later than ninety
(90) days prior to instituting use of any additional biocide used in cooling systems which
may be toxic to aquatic life other than those previously reported to the Division of Water
Quality. Such notification shall include completion of Biocide Worksheet Form 101 and a
map locating the discharge point and receiving stream.

• 9/26/97 ATC issued for construction of an additiona10.01 MGD plant (Engineer's
Cert. received 4/28/98).
Basinwide Plan Information:
• This sub -basin (03-07-02) contains 2 impaired waters listed on the '98 303(d) list —
Little Beaver and Endicott Creeks.
• Ambient station USGS 02112250 is located just upstream of this discharge. This
segment of the Yadkin is Support Threatened and is affected by sediment. It
received a Good -Fair bioclassification in 1996.
• No particular strategies for this area of the watershed,, aside from identifying
specific sources of sedimentation.
Correspondence:
• 3 Flow and 2 BOD violations in '96.
• 9/23/97 Compliance Inspection resulted in a Notice of Deficiency for BOD and
TSS violations in `96297.
• 3 BOD and 1 TSS violations in '97.
• Assessment of civil penalty on 8/28/98 for BOD violations in 5/98.
• 1 BOD and TSS violation in '98.
DMR Review:
• Flows are variable, normally 0.005-0.015 MGD.
• Temps. and pHs are okay. No TRC monitoring at 002.
• BOD and TSS variable over entire range of limit (to 30 mg/1). Ammonia also
variable; often < 10 mg/1.
General Comments:
No process wastewater is discharged from this plant— only sanitary and non -
contact cooling water.
Per Steve Pellei's 12/7/98 Memo regarding nutrient monitoring in the Yadkin
basin, this facility should be given quarterly TN and TP monitoring at 002 (001 in
non -contact cooling water).
Expiration date to be assigned to this permit is December 31, 2003.
