Dear Mr. Ward:
A letter of request for Authorization to Construct was received
January 27, 1986, by the Division and final plans and specifications for the subject
project have been reviewed and found to be satisfactory. Authorization is hereby
granted for the construction of a 20,000 GPD wastewater treatment facility consisting
of a bar screen with comminutor, five 5000 gallon aeration tanks, a 6,200 gallon
clarifier, a 5,000 gallon aerated sludge holding tank, a tertiary filter with two
4-.foot.diameter filter cells and backwash pumps, a hypochlorinator with chlorine
contact occurring in the clearwell of the tertiary filter, a recording flow meter, a
1,160 8allon post aeration tank, dual 148 CFM blowers, and dual sludge drying beds
each measuring 25.6 feet by 12.8 feet to serve Amhurst Subdivision with a treated
discharge into an unnamed tributary of Middle Creek.
This Authorization to Construct is issued in accordance with Part III
paragraph C of the NPDES Permit NO. NC0061638 issued June 11, 1986, and shall be
subject to revocation unless the wastewater treatment facilities are constructed in
accordance with the conditions and limitations specified in Permit No. NC0061638.
The Permittee must employ a certified wastewater operator in
accordance with Part III paragraph D of the referenced permit.
The sludge generated from these treatment facilities must be disposed
of in accordance with G.S. 143.215.1 and in a manner approvable by the North Carolina
Division of Environmental Management.

notified in advance of operation of the installed system so that an in -place
inspection can be made. Such notification to the Regional Supervisor shall be made

during normal office hours from 8:00 a.m. until 5:00 p.m. on Monday through Friday,
excluding State Holidays.
In event the facilities fail to perform satisfactorily in meeting its
NPDES permit effluent limits, the permittee shall take such immediate corrective
action as may be required by this Division, including the construction of additional
wastewater treatment and disposal facilities.
Prior to reaching 80% of the treatment plant's 20,000 GPD capacity the
permittee shall submit plans and specifications along with a letter requesting
authorization to construct additional facilities. Prior to exceeding 80% of the
treatment plants 20,000 GPD capacity the Permittee shall construct the additional
facilities.
