Changes for Renewal. The Division understands that you have made no significant changes in treatment process during the last permit cycle. NUTRIENT CONDITION [G.S. 143-215.3(a)(2)]; [G.S. 143-215.66] The Division may modify, or revoke and reissue this permit to include additional nutrient effluent limitations if: 1. significant nutrient problem -conditions arise in the receiving stream; 2. local action by the Permittee does not sufficiently reduce nutrient loading; or 3. by further study, the Division determines the need for stricter nutrient control. A. (3.) ELECTRONIC REPORTING - DISCHARGE MONITORING REPORTS [G.S. 143-215.1(b)] Proposed federal regulations require electronic submittal of all discharge monitoring reports (DMRs) and specify that, if a state does not establish a system to receive such submittals, then Permittees must submit DMRs electronically to the Environmental Protection Agency (EPA). NOTE: This special condition supplements or supersedes the following Sections within Part II of this permit (Standard Conditions for NPDES Permits): • Section B. (11.) • Section D. (2.) • Section D. (6.) • Section E. (5.) Signatory Requirements Reporting Records Retention Monitoring Reports 1. Reporting [Supersedes Section D. (2.) and Section E. (5.) (a)1 Beginning no later than March 1, 2016, 270 days from the effective date of this permit, the Permittee shall submit discharge monitoring data electronically using the NC DWR's Electronic Discharge Monitoring Report (eDMR) internet application. Monitoring results obtained during the previous month(s) shall be summarized for each month and submitted electronically using eDMR. The eDMR system allows permitted facilities to enter monitoring data and submit DMRs electronically using the internet. Until such time that the state's eDMR application is compliant with EPA's Cross -Media Electronic Reporting Regulation (CROMERR), Permittees will be required to submit all discharge monitoring data to the state electronically using eDMR and will be required to complete the eDMR submission by printing, signing, and submitting one signed original and a copy of the computer printed eDMR to the following address: NC DENR / DWR / Information Processing Unit ATTENTION: Central Files / eDMR 1617 Mail Service Center Page 4 of 6 Permit NCO0623 83 Raleigh, North Carolina 27699-1617 If a Permittee is unable to use the eDMR system due to a demonstrated hardship, or due to the facility being physically located in an area where less than 10 percent of the households have broadband access, then a temporary waiver from the NPDES electronic reporting requirements may be granted and discharge monitoring data may be submitted on paper DMR forms (MR 1, 1.1, 2, 3) or alternative forms approved by the Director. Duplicate signed copies shall be submitted to the mailing address above. Requests for temporary waivers from the NPDES electronic reporting requirements must be submitted in writing to the Division for written approval at least sixty (60) days prior to the date the facility would be required under this permit to begin using eDMR. Temporary waivers shall be valid for twelve (12) months and shall thereupon expire. At such time, DMRs shall be submitted electronically to the Division unless the Permittee re -applies for and is granted a new temporary waiver by the Division. Signatory Requirements [Supplements Section B. (11.) (b) and Supersedes Section B. (11.) (d)1 All eDMRs submitted to the permit issuing authority shall be signed by a person described in Part II, Section B. (11.)(a), or by a duly authorized representative of that person as described in Part II, Section B. (I 1.)(b). A person, not an assigned position, must be delegated signatory authority for eDMR reporting purposes. For eDMR submissions, the person signing and submitting the DMR must obtain an eDMR user account and login credentials to access the eDMR system. A review of the effluent data Qan201 I-Mar2015) shows regular discharge typical of 100%-domestic multiple dwelling residences. This discharge was approved for Water Supply (WS) waters by Britt Setzer (MRO Public Water Supply (Jan2010). Compliance Summary - BIMS compliance record shows all parameters reported in compliance with permit limits with minor exceptions. The record shows multiple erroneous violations for TRC [exceedences of state standard < 50 µg/L - no action BPJ]. A total of six (6) violations were recorded in 2012 with two (2) proceeding to enforcement for BOD-5 and fecal coliform, but no violations since December 2012 [see attached summary; Marcia Allocco (MRO) reviewer)].
