It looks like Van Medlock was listed as the Owner, which means we need a form to change that. I just need new contact

I would like to report that previous permit contacts for this site, Alisa Hatmaker and Van Medlock, are no longer
affiliated with Rogers Group.
Please update your records to include me as the new contact for permitting, reporting, fee invoices, etc.
If you are not the right person to send this to, may I ask that you please forward as needed and cc to me when you do?
I also wanted to check with you on the requirements for this site. An exemption was granted [attached; and the site is
still dormant].
I also see in the reissued general permit that we need to recertify dormancy:
D-. Renewal of Dormant Status
The permittee shall self -recertify that Dormant Status conditions are being maintained
annually by completing a Dormant Status Certification Form (available at
littps.-Z deg.nc. ov ahc)r_tt/divisions energ -mineral-land-resources npd-es-trid-u t ' l-
stormwater). In this farm, the permittee will certify that the facility is still temporarily
inactive and all portions of the site where clearing, grading and/or excavation activities
have occurred are stabilized with permanent vegetation. This documentation shall be ]cep
with the SWPPP and available for inspection upon request. The permittee must request
reissuance of Dormant Status in writing by submitting a written request to the Division's
Central Office within th.iM (0) days prior to the expiration of this General Permit to
maintain Parrnant Status.
However, I did not see the form at the link provided. If I missed, I apologize. Can you send a link or an attachment with
the Dormant Status Certification Form?
