This inspection was conducted to determine whether the facility is being operated and maintained in compliance with NPDES Permit No. NC0074306. Mr. Jesse Hill (ORC) and Mr. Jason Webb (BORC) were present and assisted in the inspection. Ms. Armeni discussed the possibility of rescinding the NPDES permit with Mr. Hill and Mr. Webb because the WTP has not had a discharge from the settling basins since 1996. Ms. Armeni confirmed with Mr. Sonny Penson (Public Works Director) that the Town would like to keep this permit. The valve from the wet well to the effluent pipe is closed off. The decant water from the basins is pumped back to the head of the facility. The basins are alternated to allow solids to dry for removal. Solids are removed and land applied by Southern Soil Builders. Access is not being maintained to the outfall. The ORC stated if they needed to discharge they would have the Town staff clear a path to the discharge pipe and sample as needed. Mr. Webb indicated that an eventual goal would be to install a pump station and connect the WTP to the Forest City Collection System. The receiving stream is an Unnamed Tributary to the Second Broad River. Town staff report that the discharge point is a dry ditch. IWC would be 100% if the facility ever did discharge. Has the permittee submitted a new application? Is the facility as described in the permit? # Are there any special conditions for the permit? Is access to the plant site restricted to the general public? Is the inspector granted access to all areas for inspection? There are no special conditions listed within the current permit. Visitation and reporting requirements have been waived since 2008 because the facility has not discharged since 1996. Mr. Webb indicated that they would submit the renewal application as soon as possible. Comment: Operations & Maintenance Yes No NA NE Is the plant generally clean with acceptable housekeeping? Does the facility analyze process control parameters, for ex: MLSS, MCRT, Settleable Solids, pH, DO, Sludge Judge, and other that are applicable? Comment: De-chlorination Yes No NA NE Type of system ? Is the feed ratio proportional to chlorine amount (1 to 1)? Is storage appropriate for cylinders? # Is de-chlorination substance stored away from chlorine containers? Are the tablets the proper size and type? The backwash water is not dechlorinated prior to entering the settling basins. If the facility ever discharges to surface water, a means of dechlorination would need to be determined prior. Comment: Are tablet de-chlorinators operational? Number of tubes in use? Comment: Flow Measurement - Effluent Yes No NA NE # Is flow meter used for reporting? Is flow meter calibrated annually? Is the flow meter operational? (If units are separated) Does the chart recorder match the flow meter? Are the receiving water free of foam other than trace amounts and other debris? If effluent (diffuser pipes are required) are they operating properly? The ORC reported that if the WTP ever needed to discharge, Town staff would clear a path to the outfall. Comment: Effluent Sampling Yes No NA NE Is composite sampling flow proportional? Is sample collected below all treatment units? Is proper volume collected? Is the tubing clean? # Is proper temperature set for sample storage (kept at less than or equal to 6.0 degrees Celsius)? Is the facility sampling performed as required by the permit (frequency, sampling type representative)? Waiver of reporting and visitation requirements as of 2008. No discharge since 1996. All decant water is recycled to the head of the WTP. Comment: Page#4 DocuSign Envelope ID: 5F6CC700-F769-474C-B12C-6D14E3A47EDC
