Legal documentation of transfer of ownership (such as relevant pages of a deed or a bill of sale) is
required for an ownership change request. Articles of incorporation are not sufficient for an
ownership change but can be provided for a name change.
Information to document facility, industrial activities, stormwater discharges, or outfall changes
as noted in item IV above (if appropriate)
Why is this information needed?
Regulations in 40 CFR §122.63 allow for minor modifications to NPDES permits for a change of
ownership or operational control of a facility, provided that information supports that no other change
in the permit are necessary.
Why does this form need to be mailed in?
Permittees and applicants must fulfill signatory requirements in the NPDES federal
regulations in 40 CFR §122.22 (please see those regulations for guidance). Until NCDMs
electronic submission process meets Cross -Media Electronic Reporting (CROMERR)
requirements, this original signed (not digital signature) form must be mailed to the
address below. The uploaded copy is stored as part of the permit record in the Division's
digital repository.

Nick Iknayan needs to be the owner affiliate for the two permits listed below. He is the new plant manager
(formerly Chris McGill). The plant manager at Youngsville is Josh Perry. You can reach out to him about the
Youngsville permit but Huntersville is not affiliated with Youngsville.
This should be a swap of name for name. Chris was not ever responsible for Youngsville.
I will continue to remain as the permit contact and facility contact so I have the authority to sign and submit
reporting. I'm not sure how the Youngsville facility got tied into our paperwork when I submitted an owner
affiliate change for the NCG030664 SW permit to be entered into the eDMR system.

Please advise if we need to discuss further.

HI Jamie,
The current owner affiliation for this organization is Chris McGill. Is he in charge of all three facilities, if not, who
oversees all three facilities?
What we can do is make Nick the facility contact, permit contact, and/or have signatory authority.
• If you want to designate people specific to a site, they should be entered as a Facility Contact. They will
appear on permit summaries for wastewater permits associated with the facility, too.
• If they are only specific to a permit, they should be entered as a Permit Contact. For example, if they do
the stormwater permitting but not the wastewater permitting.
• If Nick is the person who will be signing off on permit related paperwork, then he can have designated
signature authority.
