Division personnel have reviewed and approved your application for renewal of the subject permit.
Accordingly,we are forwarding the attached NPDES discharge permit. This permit is issued pursuant to the
requirements of North Carolina General Statute 143-215.1 and the Memorandum of Agreement between North
Carolina and the U.S.Environmental Protection Agency dated October 17,2007(or as subsequently amended.)
This final permit includes no major changes from the draft permit sent to you on October 7,2009.
The final permit does contain the same changes recognized in the draft permit which differed from
your current permit as follows:
• Carbon Tetrachloride,Tetrachloroethene and Trichloroethene have been reported below detectable levels
since 2005. All three parameters have been removed from the permit.
• Parameters found in the effluent since January 2007 include: 1,1-Dichloroethylene, 1,1-Dichloroethane,
cis 1,2 Dichloroethene,and 1,1,1 Trichloroethane.
o 1,1-Dichloroethylene will remain in the permit and monthly monitoring will be maintained. 1,1-
Dichloroethane and cis 1,2 Dichloroethene will be added to the permit and monthly monitoring will
be required.
o Reasonable potential exists for 1,1,1 Trichloroethane to violate Water Quality Standards.
1,1,1 Trichloroethane was added to the permit and monthly monitoring will be required.
A Daily maximum and a Monthly average limit of 4.4µg/L will be placed on 1,1,1
Trichloroethane. .
• The wording in Section A. (2.)has been changed and the following sentence added: If the
Permittee monitors any pollutant more frequently then required by this permit, the

All previous NPDES Permits issued to this facility,whether for operation or discharge are hereby
revoked. As of this permit issuance,any previously issued permit bearing this number is no longer
effective.Therefore,the exclusive authority to operate and discharge from this facility arises under the
permit conditions,requirements, terms,and provisions included herein.
Trinity American Corporation is hereby authorized to:
1. Continue to operate an existing 0.072 MGD groundwater remediation system consisting of the

A (1). EFFLUENT LIMITATIONS AND MONITORING REQUIREMENTS
Beginning on the effective date of this permit and lasting until expiration,the Permittee is authorized to
discharge treated groundwater from Outfall 001. Such discharges shall be limited and monitored by
the Permittee as specified below:

A.12.) CHRONIC TOXICITY PERMIT LIMIT(QRTRLY) •
The effluent discharge shall at no time exhibit observable inhibition of reproduction or significant mortality to
Ceriodaphnia dubia at an effluent concentration of 90%.
The permit holder shall perform at a minimum,quarterly monitoring using test procedures outlined in the"North
Carolina Ceriodaphnia Chronic Effluent Bioassay Procedure,"Revised February 1998,or subsequent versions or
"North Carolina Phase II Chronic Whole Effluent Toxicity Test Procedure"(Revised-February 1998)or subsequent
versions.The tests will be performed during the months of January,April,July and October. Effluent sampling
for this testing shall be performed at the NPDES permitted final effluent discharge below all treatment processes.
If the test procedure performed as the first test of any single quarter results in a failure or ChV below the permit
limit,then multiple-concentration testing shall be performed at a minimum,in each of the two following
months as described in"North Carolina Phase II Chronic Whole Effluent Toxicity Test Procedure"(Revised-
February 1998) or subsequent versions.
The chronic value for multiple concentration tests will be determined using the geometric mean of the highest
concentration having no detectable impairment of reproduction or survival and the lowest concentration that does
have a detectable impairment of reproduction or survival. The definition of"detectable impairment,"collection
methods,exposure regimes,and further statistical methods are specified in the"North Carolina Phase II Chronic
Whole Effluent Toxicity Test Procedure" (Revised-February 1998)or subsequent versions.
All toxicity testing results required as part of this permit condition will be entered on the Effluent Discharge
Monitoring Form(MR-1)for the months in which tests were performed.If reporting pass/fail results using the
parameter code TGP3B,DWQ Form AT-1 (original) is sent to the below address. If reporting Chronic Value
results using the parameter code THP3B,DWQ Form AT-3(original)is to be sent to the following address:

Completed Aquatic Toxicity Test Forms shall be filed with the Environmental Sciences Section no later than 30
days after the end of the reporting period for which the report is made.
Test data shall be complete,accurate,include all supporting chemical/physical measurements and all
concentration/response data,and be certified by laboratory supervisor and ORC or approved designate signature.
Total residual chlorine of the effluent toxicity sample must be measured and reported if chlorine is employed for
disinfection of the waste stream.
Should there be no discharge of flow from the facility during a month in which toxicity monitoring is required,the
permittee will complete the information located at the top of the aquatic toxicity(AT)test form indicating the
facility name,permit number,pipe number,county,and the month/year of the report with the notation of"No
Flow"in the comment area of the form. The report shall be submitted to the Environmental Sciences Section at the
address cited above.
Should the permittee fail to monitor during a month in which toxicity monitoring is required,monitoring will be
required during the following month.
Should any test data from this monitoring requirement or tests performed by the North Carolina Division of Water
Quality indicate potential impacts to the receiving stream,this permit may be re-opened and modified to include
alternate monitoring requirements or limits.
If the Permittee monitors any pollutant more frequently then required by this permit,the results of such
monitoring shall be included in the calculation and reporting of the data submitted on the DMR and all AT Forms
submitted.
NOTE: Failure to achieve test conditions as specified in the cited document,such as minimum control organism
survival,minimum control organism reproduction,and appropriate environmental controls,shall constitute an
• invalid test and will require immediate follow-up testing to be completed no later than the last day of the month
following the month of the initial monitoring.

Summary
This is a minor permit renewal for an existing 0.072 MGD groundwater treatment system
utilizing air stripping and carbon filters to remove solvent contaminants prior to surface
water discharge. The treatment system was originally permitted in December 1997.
Monthly effluent data since January 2005 have shown no detections (all<1 µg/L) for three

of the four monitored solvents(Carbon Tetrachloride,Tetrachloroethene and
Trichloroethene). All three parameters were removed from the permit.
Parameters found in the effluent during the last permit cycle and since January 2007
include: 1,1-Dichloroethylene, 1,1-Dichloroethane,cis 1,2 Dichloroethene,and 1,1,1
Trichloroethane. A Reasonable Potential Analysis was performed on each parameter and
the results are attached. It was recommended that all four of the parameters found in the
effluent be monitored in the permit and limits be placed on 1,1,1 Trichloroethane.
In addition,quarterly chronic toxicity test results have passed all events since
1998. Thus,effluent data shows that the treatment system is performing satisfactorily,and
not expected to adversely impact the aquatic habitat.
The facility discharges to an onsite fire protection pond which overflows to an
unnamed tributary to Caraway Creek in the Yadkin-Pee Dee River Basin. The receiving
stream is considered zero-flow,is not 303(d) listed,and has no specific management
strategies. NC WRC has noted that Caraway Creek provides habitat for several freshwater
mussels,which are state-listed threatened,special concern,or significantly rare.
The Permittee submitted their NPDES permit renewal application(Short Form C-
GW) on 08/06/08. The current permit expired on 1/31/09. The permit has been
administratively extended until a new permit is issued. NPDES recommends renewal of the
current permit with the changes noted above.
Compliance Summary
A review of the facility's effluent data was conducted for the period January 2005 through
June 2009.The facility had no limit violations. The Permittee provided a summary of
monthly effluent results on 15 parameters sampled 2005 through 2008. The average flow
reported between 2006 and June 2009 was .004 MGD with a maximum flow of 0.0065 MGD.
Facility passed 18 of 18 chronic toxicity tests between 2005 and 2009.

FYI -On June 4th I talked to Sue Robbins with the Inactive Hazardous Waste Branch of the Division of Waste
Management,Wilmingon Office. She indicated that she was going to wade through the Glenola file, look at the data
and issue a letter to Trinity American in the coming weeks on how to proceed (closure or continuing to monitor). Still
haven't heard anything.

I am working on the Glenola remediation Site permit renewal(NPDES NC0086029). In BIMS I can see you did an
inspection on 3-25-09. We do not have a copy of the inspection here in the NPDES permit or central office files.Could
you please(or whoever)send or scan a copy of it over to me. Also if a more recent one has been performed could that
be sent as well.Other than the reporting issue(which they still seem to be doing) I do not see any significant problems
with this facility. Please let me know if that is correct and anything you think I should be informed of while drafting this
permit.

I hope all is well. It is time to move forward with Glenola's permit renewal and I am interested in getting a summary
of your effluent data as required for permit renewal.The current NPDES permit NC0086029 requires monthly sampling
of:
1,1—Dichloroethylene
Carbon Tetrachloride
Tetrachloroethene
Trichloroethene
I can see you report on several other parameters other than those listed in your permit. This is fine as long as you
always report on the ones listed in the permit as well.At a minimum, please supply me with a 3 year summary of the
analytical results for the parameters listed in the permit and several other key pollutants which were found in
detectable levels during that time frame. Be sure not to list a result as non-detectable, please list is as<XXX(whatever
the detectable level was).

I am working on the Glenola remediation Site permit renewal(NPDES NC0086029). In BIMS I can see you did an
inspection on 3-25-09. We do not have a copy of the inspection here in the NPDES permit or central office files.Could
you please(or whoever)send or scan a copy of it over to me. Also if a more recent one has been performed could that
be sent as well.Other than the reporting issue(which they still seem to be doing) I do not see any significant problems
with this facility. Please let me know if that is correct and anything you think I should be informed of while drafting this
permit.

I am reviewing the permit renewal application you submitted for the Glenola
Remediation Site NC0086029 on August 4, 2008. Everything is in order except I can
not find a summary of the effluent data. Please submit a summary of the most recent
analytical results (effluent data) containing the maximum values for each chemical
detected. At a minimum, this should include a review of sampling performed over the
last 3-5 years.

Subject Request to Renew the Operating Permit for the Groundwater Remediation System
Dear Mrs. Sprinkle:
On behalf of Trinity American Corporation (TAC), Geoscience and Technology, P.A. (GeoSci)
respectfully submits the following information and documents related to a request for renewing the
operating permit for the referenced Groundwater Remediation System.
The original and two copies of the `NPDES Permit Application-Short Form C-GW are enclosed. A
copy of a letter authorizing GeoSci to act as TAC's agent in this matter is attached. Eric Drye,
President of TAC signed the authorization letter. A Site Location/Topographic map showing the
general location of the facility and the discharge is attached as Figure 1.A detailed site map with the
discharge location is included as Figure 2 and is an adaptation of a 2007 aerial photograph from the
Randolph County GIS website.
There have been no physical changes in the facility since issuance of the last permit. Mr. John
Hailey,former president of TAC passed away last year and Mr. Drye is now the president.
No sludge is generated by the operation of this system; therefore, there is no sludge management
plan submitted with this application.The remediation system consists of a "shallow-tray" aeration
component and a bank of two carbon canisters for polishing prior to discharge.
This system requires an Operator in Responsible Charge (ORC) and a Backup ORC. Operations
agreements have been in place since November 2007 and are still current. The ORC is Quentin
Campbell, 3336 Becks Church Road, Lexington, NC, Physical/Chemical Operator #986736. The
