Accordingly,we are forwarding the attached NPDES discharge permit. This permit is issued pursuant to the requirements of North Carolina General Statute 143-215.1 and the Memorandum of Agreement between North Carolina and the U.S.Environmental Protection Agency dated October 17,2007(or as subsequently amended.) This final permit includes no major changes from the draft permit sent to you on October 7,2009. The final permit does contain the same changes recognized in the draft permit which differed from your current permit as follows: • Carbon Tetrachloride,Tetrachloroethene and Trichloroethene have been reported below detectable levels since 2005. All three parameters have been removed from the permit. • Parameters found in the effluent since January 2007 include: 1,1-Dichloroethylene, 1,1-Dichloroethane, cis 1,2 Dichloroethene,and 1,1,1 Trichloroethane. o 1,1-Dichloroethylene will remain in the permit and monthly monitoring will be maintained. 1,1- Dichloroethane and cis 1,2 Dichloroethene will be added to the permit and monthly monitoring will be required. o Reasonable potential exists for 1,1,1 Trichloroethane to violate Water Quality Standards. 1,1,1 Trichloroethane was added to the permit and monthly monitoring will be required. CHRONIC TOXICITY PERMIT LIMIT(QRTRLY) • The effluent discharge shall at no time exhibit observable inhibition of reproduction or significant mortality to Ceriodaphnia dubia at an effluent concentration of 90%. The permit holder shall perform at a minimum,quarterly monitoring using test procedures outlined in the"North Carolina Ceriodaphnia Chronic Effluent Bioassay Procedure,"Revised February 1998,or subsequent versions or "North Carolina Phase II Chronic Whole Effluent Toxicity Test Procedure"(Revised-February 1998)or subsequent versions.The tests will be performed during the months of January,April,July and October. Effluent sampling for this testing shall be performed at the NPDES permitted final effluent discharge below all treatment processes. If the test procedure performed as the first test of any single quarter results in a failure or ChV below the permit limit,then multiple-concentration testing shall be performed at a minimum,in each of the two following months as described in"North Carolina Phase II Chronic Whole Effluent Toxicity Test Procedure"(Revised- February 1998) or subsequent versions. The chronic value for multiple concentration tests will be determined using the geometric mean of the highest concentration having no detectable impairment of reproduction or survival and the lowest concentration that does have a detectable impairment of reproduction or survival. The definition of"detectable impairment,"collection methods,exposure regimes,and further statistical methods are specified in the"North Carolina Phase II Chronic Whole Effluent Toxicity Test Procedure" (Revised-February 1998)or subsequent versions. All toxicity testing results required as part of this permit condition will be entered on the Effluent Discharge Monitoring Form(MR-1)for the months in which tests were performed.If reporting pass/fail results using the parameter code TGP3B,DWQ Form AT-1 (original) is sent to the below address. Test data shall be complete,accurate,include all supporting chemical/physical measurements and all concentration/response data,and be certified by laboratory supervisor and ORC or approved designate signature. Total residual chlorine of the effluent toxicity sample must be measured and reported if chlorine is employed for disinfection of the waste stream. Should there be no discharge of flow from the facility during a month in which toxicity monitoring is required,the permittee will complete the information located at the top of the aquatic toxicity(AT)test form indicating the facility name,permit number,pipe number,county,and the month/year of the report with the notation of"No Flow"in the comment area of the form. The report shall be submitted to the Environmental Sciences Section at the address cited above. Should the permittee fail to monitor during a month in which toxicity monitoring is required,monitoring will be required during the following month. Should any test data from this monitoring requirement or tests performed by the North Carolina Division of Water Quality indicate potential impacts to the receiving stream,this permit may be re-opened and modified to include alternate monitoring requirements or limits. If the Permittee monitors any pollutant more frequently then required by this permit,the results of such monitoring shall be included in the calculation and reporting of the data submitted on the DMR and all AT Forms submitted. A Daily maximum and a Monthly average limit of 0.0044 mg/L will be placed on 1,1,1 Trichloroethane in order to protect NC Water Quality Standards. , Summary This is a minor permit renewal for an existing 0.072 MGD groundwater treatment system utilizing air stripping and carbon filters to remove solvent contaminants prior to surface water discharge. The treatment system was originally permitted in December 1997. Monthly effluent data since January 2005 have shown no detections (all<1 µg/L) for three 4 NPDES Permit Fact Sheet- 10/06/09 Glenola Remediation Site • Page 2 NC0086029 of the four monitored solvents(Carbon Tetrachloride,Tetrachloroethene and Trichloroethene). All three parameters were removed from the permit. Parameters found in the effluent during the last permit cycle and since January 2007 include: 1,1-Dichloroethylene, 1,1-Dichloroethane,cis 1,2 Dichloroethene,and 1,1,1 Trichloroethane. A Reasonable Potential Analysis was performed on each parameter and the results are attached. It was recommended that all four of the parameters found in the effluent be monitored in the permit and limits be placed on 1,1,1 Trichloroethane. In addition,quarterly chronic toxicity test results have passed all events since 1998. Thus,effluent data shows that the treatment system is performing satisfactorily,and not expected to adversely impact the aquatic habitat. The facility discharges to an onsite fire protection pond which overflows to an unnamed tributary to Caraway Creek in the Yadkin-Pee Dee River Basin. The receiving stream is considered zero-flow,is not 303(d) listed,and has no specific management strategies. NC WRC has noted that Caraway Creek provides habitat for several freshwater mussels,which are state-listed threatened,special concern,or significantly rare. The Permittee submitted their NPDES permit renewal application(Short Form C- GW) on 08/06/08. The current permit expired on 1/31/09. The permit has been administratively extended until a new permit is issued. NPDES recommends renewal of the current permit with the changes noted above. Compliance Summary A review of the facility's effluent data was conducted for the period January 2005 through June 2009.The facility had no limit violations. The Permittee provided a summary of monthly effluent results on 15 parameters sampled 2005 through 2008. The average flow reported between 2006 and June 2009 was .004 MGD with a maximum flow of 0.0065 MGD. Facility passed 18 of 18 chronic toxicity tests between 2005 and 2009.
