Accordingly, we are forwarding the attached NPDES permit. This permit is issued pursuant to the requirements of North Carolina General Statute 143-215.1 and the Memorandum of Agreement between North Carolina and the U.S. Environmental Protection Agency dated October 15, 2007 (or as subsequently amended). The final permit contains the following changes from your previous permit: • As a result of the reasonable potential analysis the limit for 1,1,1 Trichloroethane were removed from the permit. • The permit includes a Special Condition [See Part I, A. (3.)] to address the Environmental Protection Agency's (EPA's) pending requirement for you to provide electronically-submitted Discharge Monitoring Reports (eDMRs). The Division intends to fully implement the eDMR program as soon as practicable. Temporary waivers shall be valid for twelve(12)months and shall thereupon expire. At such time,DMRs shall be submitted electronically to the Division unless the permittee re-applies for and is granted a new temporary waiver by the Division. Information on eDMR and application for a temporary waiver from the NPDES electronic reporting requirements is found on the following web page: http://portal.ncdenr.org/web/wq/admin/bog/ipu/edmr Regardless of the submission method,the first DMR is due on the last day of the month following the issuance of the permit or in the case of a new facility,on the last day of the month following the commencement of discharge. 2. Signatory Requirements [Supplements Section B. (11.) (b) and supersedes Section B. (11.) (d)1 All eDMRs submitted to the permit issuing authority shall be signed by a person described in Part II, Section B. (11.)(a) or by a duly authorized representative of that person as described in Part II,Section B. (11.)(b). A person,and not a position,must be delegated signatory authority for eDMR reporting purposes. For eDMR submissions,the person signing and submitting the DMR must obtain an eDMR user account and login credentials to access the eDMR system. The treatment system was originally permitted in December 1997. The facility discharges to an onsite fire protection pond which overflows to an unnamed tributary to Caraway Creek in the Yadkin-Pee Dee River Basin. The receiving stream is considered zero-flow,is not 303(d)listed,and has no specific management strategies. NC WRC has noted that Caraway Creek provides habitat for several freshwater mussels,which are state-listed threatened,special concern,or significantly rare. The Permittee submitted their NPDES permit renewal application(Short Form C-GW) on July 9,2013. The current permit expires on January 31,2014. NPDES recommends renewal of the current permit with the changes noted above. Compliance Summary A review of the facility's effluent data was conducted for the period January 2010 through August 2013.The facility maintained excellent compliance with no reported violations.The average flow for this period was 0.0011 MGD. The maximum monthly average flow was 0.0045 MGD. NPDES Permit Fact Sheet Glenola Remediation Site Page 2 NC0086029 Reasonable Potential Analysis A reasonable potential analysis was conducted for 1,1 -Dichloroethylene,1,1 - Dichloroethane,cis-1,2 Dichloroethene,and 1,1,1 Trichloroethane. None of these parameters present reasonable potential to exceed the water quality standards or criteria.The limits for 1,1,1 Trichloroethane will be removed from the permit.See attached RPA spreadsheet for detailed results. Summary of Proposed Changes: 1. As a result of the reasonable potential analysis the limit for 1,1,1 Trichloroethane will be removed from the permit. 2. A special condition was added describing new requirements for electronic reporting of DMRs. The original and two copies of the `NPDES Permit Application-Short Form C-GW are enclosed. A copy of a letter authorizing GeoSci to act as TAC's agent in this matter is attached. Eric Drye, President of TAC signed the authorization letter. A Site Location/Topographic map showing the general location of the facility and the discharge is attached as Figure 1.A detailed site map with the discharge location is included as Figure 2 and is an adaptation of a 2007 aerial photograph from the Randolph County GIS website. Attached also is a copy of the most recent laboratory analyses for samples collected from the system on June 26, 2013. "Influent" is a sample from the incoming groundwater to the remediation system. No sludge is generated by the operation of this system; therefore,there is no sludge management plan submitted with this application. The remediation system consists of a "shallow-tray" aeration component and a bank of two carbon canisters for polishing prior to discharge. This system requires an Operator in Responsible Charge (ORC) and a Backup ORC. Operations agreements have been in place since November 2007; however,a new agreement is in place as of June 10, 2013. The former ORC, Quentin Campbell, 3336 Becks Church Road,Lexington, NC, Physical/Chemical Operator #986736, resigned as ORC as of June 1, 2013. The new ORC is Peter H. DeVries, 1781 Chelsea Place Dr.,Kernersville,NC 27284,Physical/Chemical Operator #986722.The new Backup ORC is Roy J. Wyatt, 82 Toms Lane, Glade Valley, NC 28627. Mr. Wyatt is certified as a Surface Irrigation Wastewater Operator (#997298) and operates the wastewater system at this facility. Mr. Wyatt is also a certified NC Well Driller (#3022). An application for `conditional temporary certification' for Mr. Wyatt as a Physical/Chemical Operator was sent on June 10, 2013 to the Technical Assistance and Certification Unit and the certification is still pending as of the date of this letter. Notification of the change in ORC was also sent with the certification application for Mr. Wyatt. Copies of the Operations Agreement are available on request.
